PICC置管集束化护理在脑炎颅内高压病人中的应用

摘　　要：[目的]探讨经外周静脉置入中心静脉导管(PICC)集束化护理在脑炎颅内高压中的应用效果。[方法]对22例脑炎颅内高压需要PICC导管维护的病人进行集束化护理作为观察组,将28例PICC导管维护的脑炎颅内高压病人行常规护理作为对照组。采用自行设计调查表问卷调查及使用统一PICC导管维护单记录分析导管感染、导管堵塞、血栓形成、穿刺局部皮肤过敏等并发症发生影响。[结果]PICC置管脑炎颅内高压病人接受集束化护理后导管感染、导管堵塞、血栓形成、穿刺局部皮肤过敏等并发症发生率均明显低于对照组(P<0.05);集束化组对护理感到非常满意和满意者均明显多于对照组(P<0.05)。[结论]集束化护理在PICC置管脑炎颅内高压病人维护中有显著优势,降低导管感染及导管堵塞、血栓形成、穿刺局部皮肤过敏的发生率,增加护理满意度。Objective:To probe into the application effect of cluster nursing of peripherally inserted central ve-nous catheter(PICC)for encephalitis patients with intracranial hypertension.Methods:A total of 22 encephalitis patients with intracranial hypertension requiring PICC catheter maintenance were treated with cluster nursing as observation group,while a total of 28 encephalitis patients with intracranial hypertension with PICC catheter maintenance were treated with routine nursing as control group.Catheter infection,catheter blockage,thrombosis,puncture local skin allergy and other complications were surveyed by the self-designed questionnaire and analyzed by the unified PICC catheter maintenance record.Results:The incidences of catheter infection,catheter occlusion,thrombosis,puncture local skin allergies and other complications for PICC catheter in encephalitis patients with intracranial hypertension after cluster nursing were significantly lower than those in the control group(P< 0.05).The patients in cluster group were significantly more satisfied with the nursing care than those in control group(P<0.05).Conclusions:Cluster nursing had significant advantages in maintenance of PICC catheter for encephalitis patients with intracranial hypertension,and it could reduce the incidences of catheter infection,catheter occlusion,thrombosis,puncture local skin allergies and increase nursing satisfaction.
