针刺三重穴加放血疗法治疗后循环缺血性眩晕临床观察

摘　　要：目的观察针刺三重穴加放血疗法治疗后循环缺血性眩晕的临床疗效。方法将40例后循环缺血性眩晕患者随机分为治疗组(20例)和对照组(20例),治疗组采用针刺三重穴加放血疗法治疗,对照组采用颈项针治疗,两组均连续治疗3周,观察眩晕改善情况、眩晕障碍量表评分及椎-基底动脉血流动力学指标的变化情况。结果治疗组临床疗效有优于对照组的趋势,但疗效分布比较,差异无统计学意义(P>0.05)。两组患者治疗后眩晕障碍量表评分较治疗前均显著降低(P<0.05),治疗组治疗前后各项评分降低程度较对照组更为明显(P<0.05)。两组患者治疗后椎-基底动脉血流动力学指标与治疗前比较,差异具有统计学意义(P<0.05),两组患者治疗前后各项指标差值比较,差异均有统计学意义(P<0.05)。结论针刺三重穴加放血疗法能显著改善后循环缺血性眩晕患者的临床症状,减轻眩晕障碍对日常生活的影响,改善椎-基底动脉血流动力学状态。Objective To observe the clinical efficacy of acupuncture and bloodletting therapy at triple points in the treatment of posterior circulation ischemic vertigo. Methods Forty patients with posterior circulation ischemic vertigo were randomly and equally divided into treatment group and control group. The treatment group received acupuncture and bloodletting therapy at triple points, while the control group received conventional acupuncture on the cervical spine. The treatment lasted for three consecutive weeks. The improvement in vertigo, Dizziness Handicap Inventory （DHI） score, and changes in hemody- namic parameters of the vertebral-basilar artery were evaluated. Results The treatment group had slightly better treatment outcomes than the control group. However, there was no significant difference in the outcome dis- tribution between the two groups （P〉0. 05）. Both groups had significantly reduced DHI scores after treatment （all P〈0.05） ; compared with the control group, the treatment group had significantly greater reductions in scores for all items （all P〈0.05）. Both groups had sign bral-basilar artery after treatment （all P〈0.05） ; dices after treatment between the two groups （a ficant changes in hemodynamic parameters of the verte- there were significant differences in the changes in all in- 1 P〈0. 05）. Conclusion Acupuncture and bloodletting therapy at triple points can substantially improve clinical outcomes in patients with posterior circulation is- chemic vertigo, and it can reduce the influence of vertigo disorder on daily life and improve hemodynamics of the vertebral-basilar artery.
