多趾畸形的轴向分类法及其临床应用

摘　　要：目的介绍一种新的多趾畸形分类方法轴向分类法指导临床治疗和评估预后。方法回顾性研究2009年1月到2014年12月期间于我院手外科住院治疗的55例78足多趾畸形患者。术后通过邮件、电话或面谈的形式统计所有患者矫正手术后患足疼痛、外形、功能和穿鞋这四方面出现的不良事件病例数。然后应用轴向分类法和Venn-Watson分类法对本组55例多趾畸形进行分类，并根据手术方式和随访结果，对比和讨论两种分类方法的优劣。结果轴向分类法的每种类型多趾畸形明确对应一种手术术式，同时术后出现的不良事件主要集中在分叉型和完全重复型。Venn-Watson分类法无法直接指导手术矫正，也无法有效评估手术预后。结论本研究中提出的多趾畸形轴向分类法优于以往的分类方法，对于多趾畸形的临床手术治疗和预后评估具有指导意义。Objective To present a new classification ofpolydactyly of the foot, axial classification, in order to guide clinical treatment and evaluate prognosis. Methods Retrospective study was conducted on 55 cases with 78 feet ofpolydactyly treated in our hospital from January 2009 to December 2014. The number of adverse events in the four aspects of foot pain, appearance, function and wearing shoes was collected by email, telephone or interview. Axial classification and Venn-Watson classification were used to classify 55 cases of polydactyly of the foot, and the advantages and disadvantages of the two classifications were compared and discussed according to the surgical methods and the follow-up outcomes. Results Each type of polydactyly of the foot of axial classification clearly corresponded to an operation. The adverse events mainly occurred in the bifurcation type and the complete duplication type. The Venn-Watson classification can not directly guide the surgical correction, nor can it effectively evaluate the prognosis of the operation.. Conclusion The new classification of polydactyly of the foot is better than the previous system and can provide a guidance to help determine the surgical method and prognosis evaluation.
