分期心理护理对胸心外科手术病人术后负性情绪及生活质量的影响

摘　　要：目的:探讨分期心理护理对胸心外科手术病人术后负性情绪及生活质量的影响。方法:行胸心外科手术病人128例,采用随机数字表法分为观察组65例和对照组63例。对照组给予常规护理,观察组在此基础上进行分期心理护理。连续干预2周后,比较2组病人干预前后的社会功能、睡眠质量和焦虑、抑郁程度,并比较2组病人的护理满意度。结果:2组病人干预前社会功能、焦虑、抑郁评分差异均无统计学意义(P>0.05),干预后观察组病人社会功能、焦虑、抑郁评分改善程度均明显优于对照组(P<0.01)。2组病人干预前睡眠质量、入睡时间、睡眠时间、睡眠效率、睡眠障碍、催眠药物及日间功能障碍评分差异均无统计学意义(P>0.05),干预后观察组病人各维度评分改善程度均明显优于对照组(P<0.01)。观察组病人总满意率明显高于对照组(P<0.01)。结论:分期心理护理应用于胸心外科手术病人,可有效减轻其负性情绪,改善生活质量,具有一定的临床应用价值。Objective To investigate the effects of staging psychological nursing on the negative emotion and quality of life ofpostoperative patients treated with cardiothoracic surgery. Methods：One hundred and twenty-eight patients treated with cardiothoracicsurgery were divided into the observation group（65 cases） and control group（63 cases） using random number table. The control groupand observation group were nursed with routine care and staging psychological nursing based on routine care,respectively. The socialfunction,sleep quality,anxiety and depression in two groups between before and after intervention were compared, and the nursingsatisfaction degree between two groups was compared. Results：Before nursing,there were no significant differences in the scores ofsocial function,anxiety and depression between two groups（P 〉 0. 05）. After nursing,the improvement of the social function,anxietyand depression in observation group was significantly better than that in control group （ P 〈 0. 01 ）. Before nursing, there were nosignificant differences in the sleep quality,sleep time,sleep efficiency,sleep disorders,hypnotic drug and daytime dysfunction betweentwo groups（P 〉 0. 05）. After nursing,the improvement of each dimension in observation group was significantly better than that incontrol group（P 〈 0. 01）. Conclusions：The application of staging psychological nursing in patients treated with cardiothoracic surgerycan effectively alleviate the negative emotion and quality of life in postoperative patients,which has certain clinical reference values.
