摘　　要：目的了解因企业关停并转遗留到社会上的尘肺病的发病现状,为河北省制定尘肺病防治措施提供依据。方法收集2010—2015年河北省由公共管理和社会保障机构统一安排进行诊断的尘肺病病例信息资料,并建立数据库,采用SPSS 17.0对病例的时间、地区、病种、年龄、接尘工龄、工种进行统计描述。结果 818例无企业归属的尘肺病患者,分布在5个山区贫困县;涉及病种为矽肺812例、煤工尘肺3例、石棉尘肺3例;首次诊断为壹期尘肺占39.27%,贰期尘肺占29.88%、叁期尘肺占30.85%;矽肺接尘工龄中位数为7.33 a,明显短于同期由企业认可诊断的尘肺病及2001—2012年河北省矽肺接尘工龄(P<0.05)。结论无企业归属的尘肺病患者病情重,接尘工龄短,呈地区、行业、企业聚集的特点,是目前应重点关注的人群。Objective To explore the characteristic of pneumoconiosis, that were missed in the society due to business failure or combination,in order to give the evidence for Hebei government to design appropriate policies to prevent and control pneumoconiosis. Methods The data on pneumoconiosis cases diagnosed by Hebei institutions of public administration and social security from 2010 to 2015 was analyzed,including reported time,distributions, diseases, patient age,exposure duration, job title. Results There were 818 cases of pneumoconiosis patients who were not affiliated with the enterprise, distributed in 5 poor counties. Among them 812 cases were suffered from silicosis, 3 cases from coal workers＇ pneumoconiosis,3 cases from asbestosis. The cases with pneumoconiosis stage — diagnosed firstly were accounted for 39. 27%,stage-Ⅱ accounted for 29. 88%, stage-Ⅲ accounted for 30. 85%. The median duration of the exposure to Silicosis was 7. 33 a, significantly shorter than the pneumoconiosis from enterprise recognized diagnosis in the same period and silicosis between 2001 and 2012 in Hebei Province（P 0. 05）. Conclusions The pneumoconiosis patients who were not affiliated with the enterprise have serious condition, short duration of dust exposure. It shows special characteristics in the regional, the industry,and the enterprises, which should be focus of current prevention of pneumoconiosis.
