摘 要:目的:对比研究头穴丛刺联合经验方治疗脑缺血大鼠的作用及机制。方法:选取健康大鼠随机分组,预标记48 h后行MCAO造模,治疗方法采用头穴丛刺联合补阳还五汤原方及头穴丛刺联合加味经验方分组治疗。在各时间点分别处死取材,以神经功能评定、行为学评定、病理学检查和Western blot分别测定Hes5蛋白指标,对比研究,观察头穴丛刺联合加味经验方对模型大鼠NSC的影响。结果:经过大量处方筛选发现,头穴丛刺联合补阳还五汤传统组方在加入醉魂藤、走马芹、钻秆虫等药材后,在神经功能评分、行为学功能评分方面两种治疗方法治疗脑缺血大鼠均有作用,但加味经验方作用更佳(P<0.05)。检测脑组织含水量在梗死周边区的表达Hes5蛋白,指标发现两种治疗方法治疗脑缺血大鼠均有作用,但加味经验方组作用更佳(P<0.01)。结论:头穴丛刺联合补阳还五汤传统组方加入醉魂藤、走马芹、钻秆虫等药材后作用最佳,其作用机制可能是参与影响脑梗死后神经干细胞的增殖、迁移和分化。Objective: To explore the effect and the mechanism of scalp clustering acupuncture combined with empirical formula in the treatment of cerebral ischemia( CI) in rats. Methods: MCAO was used to establish the models. The CI rats were randomly divided into group A( scalp clustering acupuncture + Buyang Huanwu decoction) and group B( scalp clustering acupuncture + empirical formula). Empirical formula was modified on the basis of Buyang Huanwu decoction,in which Zuihunteng( Asclepiadaceae Heterostemma alatum),Zoumaqin( Angelica dahurica),and Zuanganchong( Grain insects) were used. Evaluation of neurological function,assessment of behavior,examination of pathology,and expression of Hes5 protein were compared between the two groups to explore the effect and relative mechanism. Results: In terms of the scores of neurological function evaluation and behavior assessment,they were both elevated in the two groups; however,group B had better results( P < 0. 05). In terms of the expression of Hes5 protein around infarction border,it proved both treatments had therapeutic effects; treatment applied in group B was better( P < 0. 05). Conclusion: The therapy of scalp clustering acupuncture combined with modified empirical formula is more effective in treating CI rats,which mechanism may be related to its intervention in proliferation,migration,and differentiation of neural stem cell after cerebral infraction.
