纤维支气管镜下气囊导管介入治疗在复治菌阳空洞性肺结核患者中的应用

摘　　要：目的探讨纤维支气管镜下气囊导管介入治疗在复治菌阳空洞性肺结核患者中的应用效果。方法选择2015年1月至2016年6月100例复治菌阳空洞性肺结核患者作为研究对象,采用随机数表法将其分为观察组(50例)与对照组(50例)。对照组强化期2个月使用氨基水杨酸异烟肼、利福喷汀、吡嗪酰胺、丁胺卡那霉素、盐酸乙胺丁醇、左旋氧氟沙星治疗,观察组采用纤维支气管镜下气囊导管介入治疗。比较两组转阴率、病灶改善情况及不良反应。结果观察组治疗1、2个月末的痰菌转阴率均明显高于对照组,差异有统计学意义(P<0.05);治疗后,观察组空洞病灶有效率为94.00%(47/50),明显高于对照组的72.00%(36/50),差异有统计学意义(P<0.05)。结论复治菌阳空洞性肺结核患者采用纤维支气管镜下气囊导管介入治疗效果显著,能有效净化空洞、促进痰菌转阴,且无明显不良反应,可作为一种安全高效的介入疗法广泛用于临床的治疗中。Objective To investigate the application effect of balloon catheter interventional therapy under bronchofiberscope on sputum positive cavity tuberculosis. Methods One hundred patients with sputum positive cavity tuberculosis from January 2015 to June 2016 were selected as research objects,and they were divided into observation group（ n = 50）and control group（ n = 50） by random number table. Control group took isoniazid aminosalicylate,rifapentine,pyrazinamide,amikacin,ethambutol hydrochloride,levofloxacin for two months in intensive phase,and observation group took balloon catheter interventional therapy under bronchofiberscope. The negative conversion rate,lesion improvement and adverse reactions of two groups were compared. Results The sputum negative conversion rate after treatment in the first,in second month,observation group were significantly higher than those of control group（ P〈0. 05）. After treatment,the effective rate of cavitary lesion of observation group was 94. 00%（ 47/50）,which was significantly higher than of control group[72. 00%（ 36/50）,P〈0. 05） ]. Conclusions Balloon catheter interventional therapy under bronchofiberscope has significant curative effects on sputum positive cavity tuberculosis,it can effectively purify cavity,promote sputum negative conversion and has no obvious adverse reaction. It is a safe and efficient interventional therapy in clinical treatment.
