摘 要:目的 探讨血浆核因子-κB(NF-κB)、超敏C反应蛋白(hs-CRP)、全血白细胞计数(WBC)及中性杆状核粒细胞(Nst)分类计数联合检测在新生儿败血症早期诊断和治疗中的意义。方法 随机选取已确诊为细菌感染的71例新生患儿为观察组,同期收治的无合并症的母乳性黄疸早产儿61例为对照组,比较两组新生儿的血浆NF-κB、hs-CRP、WBC与Nst分类计数检测结果。结果 观察组血清NF-κB、hs-CRP、WBC、Nst分类计数的检测结果分别为(15.72±4.65)mg/L、(18.91±10.62)mg/L、(19.06±3.61)×109/L、(57.27±12.85)%,与对照组比较,差异均有统计学意义(P〈0.01)。观察组治疗前血清NF-κB、hs-CRP、WBC、Nst分类计数的阳性率分别为85.9%(61/71)、77.5%(55/71)、45.1%(32/71)、64.8%(46/71),与对照组比较差异有统计学意义(P〈0.01);治愈出院前血清NF-κB、hs-CRP、WBC、Nst分类计数检测结果分别为(3.72±1.65)mg/L、(4.25±2.61)mg/L、(8.91±2.01)×109/L、(30.06±5.15)%,较治疗前显著降低,差异有统计学意义(P〈0.01)。结论 血清NF-κB、hs-CRP、全血WBC及Nst分类计数联合检测对新生儿败血症早期诊断具有重要临床价值。Objective To investigate the clinical significance of plasma nuclear factor kappa B (NF-κB), high sensitive C reactive protein (hs-CRP), white blood count (WBC) and neutral rod granulocyte (Nst) count examination in early diagnosis and treatment of neonatal septicemia.Methods Seventy-one cases of neonatal bacterial infection were randomly selected as the observation group, and 61 cases without complications of breast milk jaundice preterm infants in the same period were selected as the control group. The plasma levels of NF-κB, hs-CRP, WBC and Nst count were compared.Results The serum levels of NF-κB, hs-CRP, WBC and Nst count in the observation group were respectively (15.72±4.65) mg/L, (18.91 ±10.62) mg/L, (19.06 ± 3.61)×109/L, (57.27±12.85)%. Compared with the control group, the differences were significant (P〈0.01). The positive rates of the serum NF-κB, hs-CRP, blood WBC and Nst count in the observation group before treatment were 85.9%(61/71), 77.5%(55/71), 45.1%(32/71), 64.8%(46/71), there were significant differences compared with the control group (P〈0.01). The serum levels of NF-κB, hs-CRP, WBC count and Nst after cured were (3.72 ±1.65) mg/L, (4.25±2.61) mg/L (8.91 ±2.01)×109/L, (30.06 ±5.15)%, which were significantly lower than those before treatment(P〈0.01).Conclusions The combined detedction of NF-κB, hs-CRP, WBC and Nst count have important clinical value for early diagnosis of neonatal sepsis.
