地黄叶总苷胶囊联合盐酸贝那普利片治疗儿童紫癜性肾炎60例疗效观察

摘　　要：目的观察地黄叶总苷胶囊联合盐酸贝那普利片治疗儿童紫癜性肾炎的临床疗效，为临床治疗提供新的治疗方法及指导。方法将60例确诊为紫癜性肾炎、临床分型为血尿和蛋白尿型的患儿随机分为两组，治疗组30例给予地黄叶总苷胶囊联合贝那普利片治疗，对照组30例单纯给予贝那普利片治疗，均治疗12周。观察两组治疗前后尿微量白蛋白、24 h尿蛋白定量、尿沉渣红细胞计数、血清白蛋白和肌酐的变化及不良反应发生情况，评价两组疗效。结果治疗组总有效率为93.33%，对照组为73.33%，两组比较差异有统计学意义（P〈0.05）。治疗后12周，两组患儿尿微量白蛋白、24 h尿蛋白定量及尿沉渣红细胞计数水平均显著降低，且治疗组低于对照组（P〈0.05）。治疗前后两组白蛋白略有增高，但差异均未见统计学意义（P均〉0.05）；治疗前后两组血肌酐比较差异未见统计学意义（P〉0.05）。结论地黄叶总苷胶囊联合贝那普利片治疗血尿和蛋白尿型紫癜性肾炎疗效优于单用贝那普利，能更显著地降低尿蛋白、减轻血尿，且无明显不良反应。Objective To investigate the effect of Dihuangye total glycoside capsule combined with Benazepril hydrochloride tablets on Henoch-Schonlein purpura nephritis （HSPN）, so as to provide new treatment and guidance for clinical treatment.Methods Sixty children with HSPN whose clinical types were hematuria and proteinuria were selected. They were randomly divided into treatment group and control group, with 30 cases in each group. The patients in control group were treated with Benazepril hydrochloride tablets for 12 weeks simply, the patients in treatment group were treated with Dihuangye total glycoside capsule combined with Benazepril hydrochloride tablets for 12 weeks. The levels of urinary microalbumin, 24 h urinary protein, red blood cells in urinary sediment, serum albumin, serum creatinine, the efficacy and side effects were observed in the two groups before and after treatment, and the curative effect of the two groups was evaluated.Results The total effective rate in treatment group （93.33%） was significantly higher than that in control group （73.33%）, P〈0.05.The urinary microalbumin, 24 h urinary protein, red blood cell in urinary sediment significantly reduced after 12 weeks of treatment in the two groups, and the treatment group lowered more significantly （P〈0.05）. Serum albumin slightly increased after treatment in the two groups, but the difference were not statistically significant （P〉0.05）. Serum creatinine was not significantly different between the two groups before and after treatment（P〉0.05）.Conclusions The efficacy of Dihuangye total glycoside capsule combined with Benazepril on HSPN of hematuria and proteinuria type proves to be better than that of Benazepril alone. This therapy can significantly reduce the urinary protein and hematuresis without any significant side effects.
