摘 要:目的探索兰索拉唑对老年髋部骨折术后并发应激性溃疡患者氧化指标的影响。方法选择南京市江宁医院消化内科在2014年1月至2015年12月收治老年髋部骨折术后并发应激性溃疡的患者94例进行研究分析,按照患者对治疗方式的选择,分为观察组49例及对照组45例。观察组给予注射用兰索拉唑,对照组给予注射用奥美拉唑,疗程7 d。比较两组患者血清中氧化指标的变化及临床体征的差异。结果治疗前,观察组与对照组患者的超氧化物歧化酶(SOD)及活性氧(ROS)含量比较差异均无统计学意义(P>0.05)。治疗后,观察组患者的SOD显著提高至(267.2±12.9)U/m L,且ROS对应性降低至(624.9±19.4)U/m L,均优于对照组,差异具有统计学意义(P<0.05)。观察组患者治疗后发生肠鸣音增强、隐血试验阳性、Hb明显降低的总发生率为2.04%,低于对照组的20.00%,差异有显著统计学意义(P<0.05)。结论兰索拉唑治疗老年髋部骨折术后并发应激性溃疡患者可明显降低ROS、提高SOD含量,降低氧化应激水平,改善溃疡的临床表现,提高治愈效果。Objective To explore effect of lansoprazole on the oxidation index in elderly patients of hip fracture with postoperative stress ulcer. Methods Ninety-four elderly patients of hip fracture with postoperative stress ulcer in Nanjing Jiangning Hospital from January 2014 to December 2015 were divided into two groups. Patients in the observation group(49 cases) were injected with lansoprazole, and those in the control group(45 cases) received omeprazole injection, for one week. The differences between the two groups in the change in serum oxidative indexes and clinical signs were observed. Results Before treatment, the oxidation indexes, superoxide dismutase(SOD) and reactive oxygen species(ROS) had no significant difference between the observation group and control group(P〈0.05).After treatment, SOD in the observation group was significantly improved to(267.2±12.9) U/m L, and ROS significantly decreased to(624.9 ± 19.4) U/m L, which were significantly better than those in the control group(P〈0.05). The total rate of increased bowel sounds, positive results from occult blood test, Hb decrease was 2.04%in the observation group after treatment, respectively, which were significantly lower than the corresponding20.00% in the control group(P〈0.05). Conclusion Lansoprazole treatment for elderly patients of hip fracture with postoperative stress ulcer can significantly reduce ROS, increase SOD, reduce oxidative stress levels and improve clinical performance of ulcers, and thus improve healing effect.
