摘 要:目的探讨妊娠期妇女应用低分子肝素(LMWH)的效果及安全性。方法选取2010-2014年医院收治的早发型子痫前期患者135例作为研究对象,随机分为观察组68例和对照组67例,观察组应用低分子肝素联合硫酸镁进行治疗,对照组采用硫酸镁进行治疗。观察两组患者的血压变化、APTT值、妊娠结局、新生儿情况等来判断两组方法的临床疗效,记录并进行比较。结果两组患者血压较治疗前均有显著下降;APTT值观察组(37.3±2.8)s与对照组(35.3±2.7)s比较差异有统计学意义(P<0.01);观察组患者尿蛋白好转率41.18%优于对照组的23.88%,差异有统计学意义(P<0.05);孕妇常见妊娠并发症的发生率两组差异无统计学意义(P>0.05)。观察组新生儿体质量(1 547.50±309.56)g与对照组(1 542.26±522.51)g差异无统计学意义(P>0.05)。结论应用低分子肝素预防妊娠期妇女子痫,取得了较好的临床效果,安全有效且不良反应较少,值得临床进一步推广研究。Objective To explore the effect and safety of low molecular weight heparin (LMWH) in pregnant women. Methods A total of 135 patients with early-onset preeclampsia treated in Huidong County Maternal and Child Health Care Hospital from 2010 to 2014 were selected, then they were divided into observation group (68 patients) and control group (67 patients) randomly. The patients in obser- vation group were treated by LMWH combined with magnesium sulfate, and the patients in control group were treated by magnesium sulfate. The changes of blood pressure, activated partial thromboplastin time (APTT), pregnancy outcomes, neonatal situations in the two groups were observed to predict the clinical effects, the results were recorded and compared. Results The levels of blood pressure in the two groups after treatment were significantly lower than those before treatment. APTT in observation group and control group were (37. 3±2. 8 ) s and (35.3±2. 7) s, respectively, there was statistically significant difference between the two groups (P〈0. 01 ) . The improvement rate of urine protein in observation group was 41.18%, which was statistically significantly higher than that in control group (23.88%) (P〈 0. 05 ). There was no statistically significant difference in the incidence rates of pregnancy complications between the two groups (P〉0. 05). Neonatal weights in observation group and control group were ( 1 547.50±309. 56) g and ( 1 542. 26±522. 51 ) g, respectively, there was no statistically significant difference between the two groups (P〉0. 05) . Conclusion The clinical effect of LMWH in prevention of early-on-set preeclampsia is good, LMWH is safe and effective, the adverse reactions are few, which is worthy of further promotion and research.
