米非司酮配伍米索前列醇结合B超监护下吸宫术治疗早期内生型切口妊娠的疗效观察

摘　　要：目的探究米非司酮配伍米索前列醇结合B超监护下吸宫术治疗早期内生型切口妊娠的临床疗效,为切口妊娠患者的临床治疗提供参考。方法选取该院2013年5月-2016年11月收治的早期内生型切口妊娠患者42例作为研究对象,根据患者采取治疗方式的不同将患者分为观察组和对照组各21例,观察组患者采取米非司酮配伍米索前列醇结合B超监护下吸宫术进行治疗,对照组采用经阴道子宫切口瘢痕妊娠病灶清除术进行治疗,观察两组患者的临床疗效。结果观察组患者的术中出血量、住院时间以及治疗费用分别为(17.5±4.9)ml,(6.3±0.5)d以及(2 687.5±187.4)元,与对照组[(23.7±5.1)ml、(8.1±0.7)d、(9 754.3±143.8)元]比较差异有统计学意义(均P<0.05),并且观察组患者不良反应的发生率明显少于对照组,两组比较差异有统计学意义(均P<0.05),观察组患者经过治疗后的生活质量评分明显高于对照组,两组比较差异有统计学意义(均P<0.05)。结论米非司酮配伍米索前列醇结合B超监护下吸宫术治疗早期内生型切口妊娠的临床疗效显著,患者在治疗后出血量较少,治疗费用较低,生活质量较高。Objective To explore the clinical effect of mifcpristone combined with misoprostol and type B ultrasound-guided uterine aspiration in treatment of early endogenous type cesarean scar pregnancy, provide reference for clinical treatment of cesarean scar pregnancy. Methods Forty-two patients with early endogenous type cesarean scar pregnancy treated in Lianyungang Municipal Maternal and Child Health Care Hospital from May 2013 to November 2016 were selected and divided into observation group and control group according to different treatment methods, 21 patients in each group. The patients in observation group were treated by mifepristone combined with misoprostol and type B ultrasound-guided uterine aspiration, and the patients in control group were treated by transvaginal scar removal surgery. The clinical effects in the two groups were observed. Results The amount of intraoperative hemorrhage, hospitalization time, and treatment cost in observation group were （ 17.5±4. 9） ml, （ 6. 3±0. 5 ） days, and （ 2 687. 5±187.4 ） Yuan, respectively. The amount of intraoperative hemorrhage, hospitalization time, and treatment cost in control group were （ 23.7±5.1 ） ml, （ 8.1±0. 7 ） days, and （ 9 754. 3±143.8 ） Yuan, respectively. There were statistically significant differences between the two groups （all P〈0. 05 ） . The incidence rate of ad- verse reactions in observation group was statistically significantly less than that in control group （P〈0. 05 ） . After treatment, the score of lifer quality in observation group was statistically significantly higher than that in control group （P〈0.05） . Conclusion The clinical effect of mifeplistone combined with misoprostol and type B ultrasound-guided uterine aspiration in treatment of early endogenous type cesarean scar pregnancy is significant, the amount of intraoperafive hemorrhage and treatment cost are low, the quality of life is high.
