摘  要:目的:了解不同剂量的维生素D对慢性阻塞性肺疾病(COPD)稳定期患者血清T淋巴细胞亚群及肺功能的影响。方法:选取COPD稳定期患者150例(COPD组),随机分为实验A组、实验B组、对照组3组,每组50例。试验组A服用阿法骨化醇软胶囊0.25μg qd,试验组B服用阿法骨化醇软胶囊0.5μg qd,对照组服用安慰剂,连续观察1年。检测T淋巴细胞亚群以及肺功能的变化。结果:A组、B组治疗后CD3+%、CD4+%、CD4+/CD8+、FEV1%prep、FVC%prep、FEV1/FVC%明显高于治疗前(P<0.05)。对于上述指标在对照组治疗前后相比无明显差异(P>0.05),A组、B组治疗后无明显差异(P>0.05)。结论:COPD稳定期患者的气道仍然存在慢性炎症,维生素D能提高细胞的免疫功能,延缓COPD患者的肺功能下降,而且与剂量无关。[Objective]To explore the Effect of vitamin D on T lymphocyte subsets and lung function in stable COPD patients. [Method]150 cases of COPD at stable stage were divided into experimental group A,experimental group B and control group equally. T-lymphocyte subsets in serum,lung functionwere detected. [Result] CD3 + % 、CD4 + % 、CD4 +/CD8 +,FVC% prep,FEV1/FVC% after therapy were higher than those before therapy in experimental group A and B( P < 0. 05),while there was no significant difference in control group( P > 0. 05),while there was no significant difference between experimental group A and B after therapy( P > 0. 05). [Conclusion]There was chronic inflammation in patients with chronic obstructive pulmonary disease( COPD) at stable stage. Vitamin D could regulate cell immunity by decrease T lymphocyte subsets in airway of patients with COPD at stable stag,reduce airway 's inflammation,then delay the declining of lung function,and this isn't related to the dose.
