摘 要:目的探讨支气管结核患者支气管肺泡灌洗液(BALF)中骨桥蛋白(OPN)和表面活性蛋白D(SPD)的水平变化及其临床意义。方法选取2016年5月至2017年5月收治的55例支气管结核的患者作为支气管结核组,55例支气管哮喘患者作为支气管哮喘组,55例健康体检者作为对照组。采用酶联免疫吸附实验(ELISA)法测定BALF中OPN和SPD水平。将患者分为痰菌阳性组和痰菌阴性组,比较两组患者的OPN和SPD水平差异。将支气管结核患者分为活动期和非活动期两组,比较两组患者的OPN和SPD水平差异。结果支气管结核组和支气管哮喘组的OPN和SPD水平均高于对照组(P<0.05),支气管结核组的OPN和SPD水平显著高于支气管哮喘组。痰菌阳性组患者的OPN和SPD水平显著高于痰菌阴性组患者。活动性支气管结核组患者的OPN和SPD水平显著高于非活动性支气管结核组患者。OPN和SPD水平与痰菌涂片和结核活动性均具有相关性(P<0.05)。结论支气管结核患者BALF中的OPN和SPD水平显著升高,且与病灶的痰菌涂片和结核活动性密切相关。Objective To study the changes and clinical significance of osteopontin and surfactant protein D in bronchoalveolar lavage fluid of patients with bronchial tuberculosis. Methods 55 cases of bronchial tuberculosis patients treated in our hospital from May 2016 to May 2017 were selected as bronchial tuberculosis group,55 cases of bronchial asthma as bronchial asthma group,and 55 cases of physical examination as control group. Enzyme linked immunosorbent assay( ELISA) was used to determine the levels of OPN and SPD in BALF. The patients were divided into sputum positive group and sputum negative group,and the difference of OPN and SPD levels between the two groups were compared. The bronchial tuberculosis patients were divided into two groups,the active group and the inactive group,and the difference of OPN and SPD levels between the active bronchial tuberculosis group and the inactive bronchial tuberculosis group was compared. Results The levels of OPN and SPD in the bronchial tuberculosis group and the bronchial asthma group were higher than that in the control group( P 〈 0. 05). The levels of OPN and SPD in the bronchial tuberculosis group were significantly higher than those in the bronchial asthma group and control group. The levels of OPN and SPD in sputum positive group were significantly higher than those in sputum negative group. The levels of OPN and SPD in the patients with active bronchial tuberculosis were significantly higher than those in the inactive endobronchial tuberculosis group. The levels of OPN and SPD were correlated with sputum smear and tuberculosis activity( P 〈 0. 05). Conclusion The levels of OPN and SPD in bronchoalveolar lavage fluid are significantly increased in patients with bronchial tuberculosis,which is closely related with the smear of sputum and the activity of tuberculosis.
