江西省婺源县农户对新农合认知与满意度的八次动态追踪调查

摘　　要：目的:为了解农户对新农合实施以来的认知变化趋势和满意度的评价,准确评价新农合的实施效果,并为其改善工作提供相应政策建议。方法:对2003年开始试点的婺源县进行追踪调查,并基于基线调查和八次追踪调查数据进行趋势分析。结果:农户参加新农合比率逐年增高,目前的参合率(99.88%)及知晓率(100%)均达到较高水平,但对具体筹资金额知晓不足;农户到定点医疗机构看过病、报销过门诊费用和住院费用的比例呈逐年上升趋势;农户认为报销公平、公正、公开的比例在第五次追踪调查时(2010年)达到峰值(82.10%),随后下降至第九次的调查最低值(53.07%),差异有统计学意义;农户对新农合医保的整体满意度评价的下降趋势有统计学意义。结论:新农合实施十多年来,其知晓率和参合率均达到较高水平,利用率也在不断提高,但农户对新农合的具体政策依旧了解不够全面和透彻,满意度不断下降,应当引起相关机构的重视,从而改进新农合实施工作的过程和方案,完善新农合医保制度。Objective To understand the trend of farmers＇ cognition and satisfaction of new rural cooperative medical system （NRCMS） after its implement in Wuyuan, Jiangxi province, and to provide the scientific basic evidences for the correct evaluation of the implementation of NRCMS and its further improvement. Methods The pilot country - Wuyuan started since 2003 was select- ed. Based on the baseline survey and eight follow - up survey data, linear trend Chi - square tests were employed to forecast the variation tendency of the farmers＇ cognition and satisfaction on NRCMS. Results The current level of insured rate（99. 88% ） and awareness rate（ 100% ） of NRCMS had reached the high standard, while the exactly insurance payment of NRCMS were blindly. Residents who used NRCMS to visited the contract hospitals and covered the outpatient cost and inpatient cost were increasingly statistically significant year by year. On the fifth follow - up survey, we found the proportion of these who were agree with the opin- ion that the NRCMS compensation was fair enough for all has reached the top degree, and then dropped to the lowest value in the latest follow - up survey （53.07 % ）. Statistically significant downward trend in overall evaluation of satisfaction of NRCMS among farmers were found in this study. Conclusion Insured rate and awareness rate, as well as utilization rate of NRCMS were enhancing gradually after its implement for more than ten years in Jiangxi. However, there still remain some problems that need be addressed by the relevant authorities.
