摘  要:目的分析各肿瘤标记物(HE4、CEA、CA125、CA19-9)在良恶性腹水中的鉴别诊断价值。方法选择陕西中医药大学第二附属医院在2012年3月至2015年8月收治的53例癌性腹水(恶性组)及46例非癌性腹水患者(良性组),检测两组患者的血清、腹水HE4、CEA、CA125、CA19-9水平,比较各肿瘤标记物水平。结果恶性组腹水中HE4、CEA、CA19-9、CA125水平均明显高于良性组腹水(P<0.05);恶性组血清HE4、CEA、CA19-9水平均明显高于良性组血清(P<0.05);两组HE4、CEA、CA125、CA19-9在腹水中的水平均明显高于血清中(P<0.05)。腹水四个肿瘤标记物联合检测灵敏度达100.00%,特异度达97.83%,血清四个肿瘤标记物联合检测灵敏度达92.45%,特异度达97.83%。结论联合检测腹水、血清HE4、CEA、CA19-9、CA125对鉴别腹水良恶性肿瘤意义巨大。Objective To investigate the differential diagnosis value of tumor markers in patients with benign and malignancy ascites. Methods From March 2012 to August 2015, 53 patients with malignant ascites(malignant group) and46 patients with non-cancerous ascites(benign group) were selected from the Second Affiliated Hospital of Shaanxi University of Chinese Medicine, the levels of HE4, CEA, CA125 and CA19-9 in serum and ascites of the two groups were detected, and the levels of each tumor marker were compared. Results The levels of HE4, CEA, CA19-9 and CA125 in ascites of the malignant group were significantly higher than those of the benign group(P〈0.05), the levels of HE4, CEA and CA19-9 in serum of the malignant group were significantly higher than those of the benign group(P〈0.05). The levels of HE4, CEA, CA125 and CA19-9 in ascites in both groups were significantly higher than those of the serum(P〈0.05). The sensitivity and specificity of joint detection was 100.00%, 97.83% respectively, and those of the serum were 92.45%,98.11%. Conclusion Joint detection of tumor markers of HE4, CEA, CA125 and CA19-9 in serum and ascites have a great sense differential diagnosis of begnign and malignant tumors.
