长柄双极人工股骨头置换术治疗老年粉碎性股骨粗隆间骨折的临床效果

摘　　要：目的探讨长柄双极人工股骨头置换术治疗老年粉碎性股骨粗隆间骨折的临床效果。方法选取2015年7月至2016年9月我院收治的老年粉碎性股骨粗隆间骨折患者65例为研究对象,对所有患者给予长柄双极人工股骨头置换术进行治疗。观察并记录患者的手术治疗优良率、手术时间、术中出血量、输血量、输血率、并发症发生情况及手术前、后的Harris评分。结果手术治疗优良率为95.38%,手术时间为(68.62±7.36)min,术中出血量为(72.17±5.93)m L,输血量为(67.15±3.68)m L,输血率为32.31%,并发症总发生率为6.15%。术后1周、1、3、6个月的Harris评分均高于术前,差异具有统计学意义(P<0.05)。结论长柄双极人工股骨头置换术治疗老年粉碎性股骨粗隆间骨折的临床效果确切,且手术操作简便、微创,可有效改善患者关节功能。Objective To investigate the clinical effect of long-acting bipolar artificial femoral head replacement surgery in elderly patients with comminuted intertrochanteric fractures. Methods Sixty-five elderly patients with intertrochanteric fractures admitted in our hospital were selected as the study objects. All the patients were treated with long-acting bipolar artificial femoral head replacement surgery. The excellent and good rate of treatment, operation time, intraoperative blood loss, blood transfusion volume, blood transfusion rate, complications and Harris scores before and after operation were compared. Results The excellent and good rate of treatment was 95.38%, operation time was（68.62±7.36） min, intraoperative blood loss was（72.17±5.93） mL, blood transfusion volume was（67.15±3.68）, the blood transfusion rate was 32.31%, and the complications rate was 6.15%. At 1 week, 1, 3, 6 months after the operation, the Harris scores of the patients were higher than those before operation, the differences were statistically significant（P〈0.05）. Conclusion The clinical effect of long-acting bipolar artificial femoral head replacement surgery in elderly patients with comminuted intertrochanteric fractures is significant. The operation is simple and minimally invasive, which can effectively improve the comminuted joint function of the patients.
