别针型带膜食管支架的动物实验研究

摘　　要：目的 :验证一种新型的带别针的Z型带膜食管支架 (以下简称别针型支架 )的安全性、稳定性和可取出性。方法 :实验犬分为A、B、C、D 4组 ,分别予放置别针型支架、放置网状记忆合金支架后再放置别针型支架、放置带倒刺的Z型带膜食管支架、放置带膜记忆合金支架。A、C、D组分别于支架放置后 1、2、4W分批处死实验犬 ,进行病理分析 ;B组用于 12W后胃镜下取出别针型支架。结果 :A组实验犬无 1例死亡 ,C组死亡 1只 ,D组死亡 2只。A、C两组病理变化以炎性反应为主 ,部份出现溃疡 ;D组病理变化表现为炎性反应、溃疡形成、粘膜缺失、组织坏死、肉芽增生、纤维化等。B组别针型支架均顺利取出。结论 :别针型Z型带膜食管支架是一种更安全的新型食管支架 ,有可靠的稳定性 ,放置后可以取出 。Objective:To prove the safety, stability of a new 'Z' type membrana-covered esophageal stent with pin, and convenience to be taken after the stent was placed.Methods: Thirty-six dogs were randomized into 3 groups, a stent was paced in every esophagus with the following different types: A group: 'Z' type membrana-covered esophageal stent with pin,C group: 'Z' type membrana-covered esophageal stent with barb, D group: Ni-Ti alloy membrana-covered esophageal stent. Every 4 dogs of each group were killed at a time by the end of 1, 2, 4 weeks. A portion of esophagus together with the stent were taken for pathology. Other 6 dogs was B group: a 'Z' type membrana-covered esophageal stent with pin was placed in every esophagus after a Ni-Ti alloy without covered esophageal stent had been placed for 4 weeks. The 'Z' type membrana-covered esophageal stents with pin were taken out from the dogs'esophagus. Results: No dead dog was found in A group and B group, one dog dead in C group and two occurred in D group. The 'Z' type membrana-covered esophageal stents with pin were taken out successfully. The mainly pathological changes were inflammatory reaction and ulcer forming partly in A group and C group, The mainly pathological changes were inflammatory reaction, ulcer forming mucosa vacancy, tissue necrosis, granulation proliferation and fibrosis in D group.Conclusions:Our data demonstrated that the 'Z' type membrana-covered esophageal stent with pin was a safe, stable and convenient stent to be taken out from the esophageal after it was placed. We recommend it to be used in clinic.
