摘 要:目的研究2型糖尿病患者血浆Apelin光密度(optical density,OD)变化水平与肺弥散功能的关系。方法选择2015-01~2015-12在山西医科大学第二医院拟行骨科手术患者90例,将受试对象按术前糖化血红蛋白(HbA_(1c))水平分为三组:Ⅰ组(HbA_(1c)>7.0%),Ⅱ组(6.5%<HbA_(1c)<7.0%),Ⅲ组(HbA_(1c)<6.5%)。分别检测各组患者的术前25%肺活量时的最大呼气流量(25%maximal expiratory flow-volume,MEF25)、一氧化碳弥散量(carbon monoxide diffusing capacity,DLCO)、最大呼气中段流量(maximal midexpiratory flow,MMEF)、最大通气量(maximal voluntary ventilation,MVV)、Apelin OD,分析肺通气功能及弥散功能指标与HbA_(1c)、FPG、Apelin OD之间的相关性。结果Ⅰ组MEF25、Apelin OD,Ⅱ组MEF25、DLCO、MMEF、Apelin OD比Ⅲ组相应指标均有所下降,差异有统计学意义(P<0.05)。三组患者肺活量(vital capacity,VC)、用力肺活量(forced vital capacity,FVC)、Apelin OD与HbA_(1c)呈负相关(P<0.05),MEF25、弥散系数(carbon monoxide diffusing capacity/alveolar air,DLCO/Va)与Apelin OD呈正相关(P<0.05)。HbA_(1c)与DLCO、MMFF、MVV呈负相关(P<0.05),空腹血糖(fasting plasma glucose,FPG)与VC、MVV呈负相关(P<0.05)。结论 2型糖尿病患者FPG愈高,HbA_(1c)水平愈高,Apelin OD值愈低,肺通气功能损害愈重,肺弥散功能可能愈差,Apelin OD与肺弥散功能呈正相关性。Objective To explore the correlation between Apelin optical density( OD) and pulmonary diffusing capacity in patients with type 2 diabetes. Methods According to preoperative glycosylated hemoglobin A1 c( HbA1c) levels,90 patients with type 2 diabetes were divided into three groups( n = 30 in each group) : group Ⅰ( HbA1c〉 7. 0%),group Ⅱ( 6. 5% HbA1c 〈7. 0%) and group Ⅲ( HbA1c 〈6. 5%). The preoperative lung function indexes were detected in patients with type 2 diabetes,including MEF25( 25% maximal expiratory flow-volume),carbon monoxide diffusing capacity( DLCO),maximal midexpiratory flow( MMEF),maximal voluntary ventilation( MVV),Apelin OD. The correlation between lung function and HbA1c,FPG,Apelin OD was analyzed. Results MEF25 and Apelin OD were lower in group Ⅰ than in group Ⅲ( P〈0. 05),and MEF25,DLCO,MMEF,Apelin OD were also lower in group Ⅱ than in group Ⅲ( P〈0. 05). The FVC,VC and Apelin OD were negatively correlated with HbA1c( P〈0. 05),while MEF25 and DLCO/Va were positively correlated with Apelin OD( P〈0. 05). DLCO,MMFF,VC and MVV were negatively correlated with HbA1c( P〈0. 05),and MVV was negatively correlated with FPG( P〈0. 05). Conclusion The higher levels of HbA1cand FPG are,the lower Apelin OD is. The more serious the lung function damage is,the worse the pulmonary diffusion function is. The level of Apelin OD is positively correlated with pulmonary diffusing capacity.
