体外受精胚胎移植妊娠后卵巢扭转临床分析

摘　　要：目的分析体外受精胚胎移植（IVF—ET）妊娠后卵巢扭转的诊治、临床处理及对妊娠结局影响。方法回顾性分析2014年1月至2016年12月本院行IVF助孕11041周期，临床妊娠6293例，其中妊娠后发生卵巢扭转患者4例。比较分析卵巢扭转发作到处理时间，卵巢大小，卵巢扭转周数，发生扭转妊娠周数等对妊娠结局影响。结果4例妊娠合并卵巢扭转患者3例行手术后一般情况良好，其中1例34周早产一男孩，现体健。1例孕37＋周剖宫产两男孩，1例孕38＋周顺产一男孩，均体健。1例腹痛1d后胚胎停育行手术。结论体外受精胚胎移植妊娠合并卵巢扭转及时发现并行手术处理，可保留宫内胎儿。Objective To analyze the clinical characteristics, diagnosis, treatment, and outcomes of ovarian torsion in pregnant women after in vitro fertilization and embryo transfer （IVF-ET）. Methods The clinical data of pregnant women with ovarian torsion after IVF-ET in our hospital from January 2014 to December 2016 was retrospectively analyzed. Data collecting included onset / treatment time of ovarian torsion, ovary volume, torsion degree, gestational weeks, and pregnancy outcomes. Results 4 cases were collected, 3 cased were intervened by laparotomy and 1 case was treated with ipsilateral salpingectomy. 3 cases continued the pregnancy after intervention; 1 case was sent to hospital at 30 hours after the onset of ovarian torsion caused by embryonic diapause. Conclusions Ovarian torsion is an emergency complication in pregnant women after IVF-ET. Timely surgery intervention can benefit the pregnancy outcomes.
