摘  要:目的研究引起腹泻的鼠伤寒沙门菌H2阴性变异株(抗原式:Ⅰ4,5,12:i:-)的耐药与分子流行病学特征。方法根据Pulse Net实验室推荐的沙门菌PFGE分型方案与药敏试验肉汤稀释法,对2015-2016年腹泻病例分离菌株进行PFGE分子分型与最小抑制浓度测定。结果该变异株主要感染1~3岁的幼儿(52.4%),检出高峰期在夏秋季。该菌株药敏试验结果显示菌株对亚胺培南和阿奇霉素敏感;四环素和氨苄西林的耐药率较高,分别为89.84%和88.77%;对不同头孢类抗生素表现不一样的耐药,对头孢唑啉、头孢噻肟、头孢他啶和头孢西丁的耐药率分别是47.06%、39.04%、13.9%和1.6%;多重耐药率为67.38%,以3种抗生素AMP-TE-CHL耐药为主,而同时对三种头孢类抗生素耐药的有25株(13.37%)。PFGE分型结果显示187株菌分为11大聚类簇,共获得109种带型,菌株间相似值为55.4%~100%,优势型别为B58(12株)和B23(9株),菌株的PFGE型别和耐药型别间没有发现有相关性。结论珠海市鼠伤寒H2阴性变异株对氨苄西林和四环素耐药比较严重,多重耐药率较高,对头孢类抗生素呈现一定水平的耐药。本地菌株呈现遗传多样性。Objective To investigate the antimicrobial resistance and molecular epidemiological characteristics of salmonella typhimurium H2 negative strains (serotype: I 4, 5, 12: i. -)isolated from diarrhea cases in Zhuhai during 2015 - 2016. Methods The broth microdilution method was used to obtain the minimum inhibition concentration of salmonella strains isolated from diarrhea cases in Zhuhai City ,2015 -2016 ,and mnlecular typing was performed according to the salmonella PFGE genotyping test recommended by Pulse Net Laboratory. Results The children of one to three years old were mainly affected by salmonella typhimurium H2 negative strains, accounting for 52.4% ,and the pathogen isolation was mainly in summer- autumn seasons during the year. The antimicrobial resistant test showed that the strains were sensitive to imipenem (IMP)and azithromycin(AZM) ,but with a high level of resistance to tetracycline (TE), ampicillin (AMP), the resistance rates were 89. 84% and 88.77%. And for different levels of resistance to cephalosporin antibiotics ( CFZ / CTX / CAZ and CFX), the resistance rates were 47.06% /39. 04% / 13.9% and 1.6%. The multiple drug resistance (MDR) of the strains was more severe,accounting for 67.38%. The predominant drug -resistant spectrum was simultaneously resistant to AMP -TE - CHL. Meanwhile,there were 25 isolates ( 13.37% ) resistant to three cephalosporins simultaneously. The 187 isolates were divided into 11 clusters and 109 PFGE types,with the similarity range of strains from 55.4% to 100% ,the predominant types were B58 and B23 ,which included 12 strains and 9 strains, respectively. And no correlation was found between the PFGE type and the resistance type of the isolates. Conclusion The antimicrobial resistance of salmonella typhimurium I-I2 negative strains in Zhuhai is more serious, especially ampicillin and tetracycline.
