摘 要:目的探讨磁共振各向同性容积扫描(CUBE)成像技术在颞叶癫痫(TLE)脑内病变检测的临床应用价值。方法搜集2015年6月至2016年9月本院临床诊断为癫痫患者30例,年龄7~65岁,平均31.7岁。均行MRI扫描,扫描序列包括矢状位T_1FLAIR,轴位T_1FLAIR、T_2WI及T_2FLAIR,冠状位3D CUBE T_2WI、T_2FLAIR及3D BRAVO。扫描后3D CUBE图像经多层面重组技术合成多方位及多种厚度的2D影像,与常规MRI对病变的显示能力进行比较,采用SPSS 17.0进行处理分析,经卡方检验,P<0.05认为差异有统计学意义。结果 30例患者中,3D CUBE重组图像发现脑内阳性病灶29个,而常规MRI发现阳性病灶15个,3D CUBE技术比常规MRI的敏感度高46.7%(P<0.05),差异有显著意义。结论由于颞叶相邻结构复杂,当致痫灶病理改变不明显时,应运用3D CUBE扫描技术,然后采取多角度及薄层高分辨2D重组图像,有利于颅内颞叶病变的检出。Objective To investigate the clinical application of CUBE magnetic resonance imaging in temporal lobe epilepsy. Methods From June 2015 to September 2016,30 cases of patients with epilepsy were studied in our hospital. The mean age of the patients was 31. 7 years,ranging from 7-65 years old. Examinations were performed with GE Signa 1. 5 THDxt scanner. The following MRI sequences were scanned: sagittal FLAIR T_1WI; axial FLAIR T_1WI,T_2WI and FLAIR T_2WI; coronal 3D CUBE T_2WI,3D CUBE T_2FLIAR,and 3D BRAVO. After scanning,multiplanar reformations were made from 3D images,including 3 mm coronal,sagittal and axial 2 D images. Data were analyzed by using SPSS 17. 0,and chi square test. Results In 30 cases,3D CUBE showed 29 lesions in the brain,but only 15 of the lesions were found in conventional MRI. The sensitivity of 3D CUBE was 46. 7% higher than that of conventional MRI. The difference between 3D CUBE and conventional MRI was statistically significant(P〈0. 05). Conclusion MRI is an important tool for diagnosis of epilepsy. We must use the multi-sequence imaging to detect the disease area because the pathological change in temporal lobes is not obvious and the adjacent structures are complex. The multi-angle thin layer high resolution CUBE sequence is in favor of detecting the temporal lesions. Appropriate treatment planning can be initiated according to anatomic location and correct diagnosis.
