摘 要:目的探讨MRI动态增强扫描技术在判定海绵窦内侧壁完整性诊断侵袭性垂体腺瘤中的应用价值。方法搜集经手术病理证实为垂体腺瘤且术前行MRI动态增强扫描的患者共132例,回顾性分析其影像学资料并进行对比性研究。结果利用MRI动态增强扫描技术判定海绵窦内侧壁完整性诊断侵袭性垂体腺瘤,其灵敏度、特异度、阳性预测值(PPV)、阴性预测值(NPV)和约登指数分别为100%、95.65%、86.36%、100%、0.957,总体上较传统评价指标更加准确和客观。结论 MRI动态增强扫描对垂体腺瘤侵袭海绵窦内侧壁的检出很敏感,是检出侵袭性垂体腺瘤极有价值的扫描方法。Objective In the present paper,the author aims to discuss the applications of dynamic enhanced MRI scanning technology which is used for diagnosing invasive pituitary adenomas. Methods 132 subjects who had been diagnosed with pituitary adenomas and had been scanned by dynamic enhanced MRI before surgery and confirmed by surgery and pathology,had their imaging data retrospectively analyzed and compared in this study. Results The result shows that compared with traditional evaluation indexes,dynamic enhanced MRI scanning technology is more accurate and more objective in the aspects of sensitivity,specificity,positive predictive value(PPV),negative predictive value(NPV) and Youden index(sensitivity 100%,specificity 95. 65%,NPV 100%,PPV 86. 36%,Youden index 0. 957) when it is used for diagnosing invasive pituitary adenomas according to the completeness of cavernous sinus medial wall. Conclusion Dynamic enhanced MRI is very sensitive for checking cavernous sinus medial wall integrity in invasive pituitary adenomas,which suggests that it is a useful scanning method of diagnosing invasive pituitary adenomas.
