益气清络方对类风湿关节炎患者miR-146a表达和Th17、Treg水平的影响

摘　　要：目的:探讨益气清络方对类风湿关节炎(RA)患者外周血单个核细胞(PBMC)中miR-146a的表达及辅助性T细胞(Th)17、调节性T细胞(Treg)水平的影响。方法:纳入60例RA患者,随机分为治疗组(n=30)和对照组(n=30),同时纳入30例健康志愿者作为健康对照组。所有患者均给予甲氨蝶呤(MTX)治疗,治疗组患者辅以益气清络方口服,对照组患者辅以安慰剂口服,两组治疗周期均为12周。分别于治疗前后检测所有患者的PBMC中miR-146a的表达及Th17、Treg水平,且采用相同方法检测所有健康志愿者的PBMC中miR-146a的表达及Th17、Treg水平。结果:(1)与健康志愿者比较,RA患者的PBMC中miR-146a的相对表达量和Th17水平均显著升高(P<0.05,P<0.01),Treg水平明显降低(P<0.01)。(2)治疗后,两组患者的PBMC中miR-146a相对表达量及Treg水平较治疗前均明显升高(P<0.05,P<0.01),Th17水平明显下降(P<0.01);且治疗组患者的PBMC中miR-146a的相对表达量及Treg水平均明显高于对照组(P<0.01),Th17水平明显低于对照组(P<0.01)。结论:益气清络方结合传统西药治疗RA的机制可能与上调miR-146a的表达,调节Th17、Treg水平相关。Objective： To investigate the effects of ＂Yiqi Qingluo Decoction＂ on the expression of miR-146a and the levels of Th17 and Treg cells in peripheral blood mononuclear cell （ PBMC ） of patients with rheumatoid arthritis （RA）. Methods ： A total of 60 patients with RA were included and randomly divided into the treatment group （ n = 30 ） and control group （ n = 30）. Meanwhile 30 healthy volunteers were selected as the healthy control group. All the patients were treated with methotrexate （ MTX）, the patients in the treatment group were orally treated with ＂Yiqi Qingluo Decoction＂ and the patients in the control group were orally treated with placebo, additionally. The treatment course was 12 weeks in both groups. The expression of miR-146a and the levels of Thl7 and Treg in PBMC of all patients were detected before and after treatment, and the same detection were performed in the healthy volunteers. Results： ①Compared with the healthy volunteers, the expression of miR-146a and the level of Thl7 in PBMC of RA patients were significantly increased （P 〈0.05, P 〈0.01 ）, and the level of Treg was obviously decreased （P 〈 0.01 ）. ②After treatment, the expression of miR-146a and the level of Treg in PBMC in both groups were obviously increased （ P 〈 0.05, P 〈 0.01 ）, and the level of Th17 was obviously decreased （ P 〈 0.01 ）. The expression of miR-146a and the level of Treg in PBMC in the treatment group were obviously higher than those in the control group （P 〈0.01 ）, and the level of Th17 in the treatment group was obviously lower than that in the control group （P 〈 0.01 ）. Conclusion： The mechanisms of ＂Yiqi Qingluo Decoction＂ combined with conventional western medicine on RA may be associated with the up-regulation of miR-146a expression and regulation of Th17 and Treg levels.
