NB-UVB联合多磺酸粘多糖乳膏治疗银屑病的疗效观察及对IL-4、IL-37、INF-γ水平的影响

摘　　要：目的探讨窄谱中波紫外线(narrow band ultraviolet,NB-UVB)联合多磺酸粘多糖乳膏治疗银屑病的临床疗效。方法将作者医院2013-02/2016-06月收治的66例银屑病患者按随机数字表法分为对照组与观察组各33例,两组均采用NB-UVB治疗,观察组在此基础上加用多磺酸粘多糖乳膏治疗,均进行银屑病皮损面积及严重程度(psoriasis area and severity index,PASI)评分,比较两组治疗效果,并检测治疗前后两组患者血清白细胞介素4(interleukin-4,IL-4)、白细胞介素37(interleukin-37,IL-37)、干扰素γ(interferon-γ,IFN-γ)、肿瘤坏死因子α(tumor necrosis factor,TNF-α)水平的变化,观察两组治疗不良反应发生情况。结果 (1)治疗后,两组PASI评分,IL-37、IFN-γ、TNF-α水平较治疗前均降低,治疗前后比较差异具有统计学意义(P<0.05);且观察组PASI评分,IL-37、IFN-γ、TNF-α水平均低于对照组,组间比较差异具有统计学意义(P<0.05)。(2)观察组治疗总有效率明显高于对照组,组间比较差异具有统计学意义(P<0.05)。(3)两组总不良反应发生率对比差异无统计学意义(P>0.05)。结论 NB-UVB联合多磺酸粘多糖乳膏治疗银屑病临床疗效肯定,同时可降低IL-37、IFN-γ、TNF-α水平,减轻患者皮损症状,且安全性高。Objective To investigate the clinical value of narrow-band ultraviolet B（NB-UVB）combined with mucopolysaccharide polysulfate cream in treatment of psoriasis.Methods Sixty-six patients with psoriasis in author＇s hospital from February 2013 to June 2016 were divided into control group and observation group by random number table with33 cases each.Two groups were treated with NB-UVB,but the observation group was additionally treated with mucopolysaccharide polysulfate cream.The psoriasis area and severity index（PASI）scores were performed,and the therapeutic effects were compared between two groups.Changes of serum interleukin 4（IL-4）,interleukin 37（IL-37）,interferonγ（IFN-γ）and tumor necrosis factorα（TNF-α）levels in two groups were detected before and after treatment.The adverse reactions in two groups were observed.Results（1）After treatment,the PASI scores,IL-37,IFN-γand TNF-αlevels were all decreased in two groups（P0.05）,and above indexes of the observation group were lower than those of the control group（P0.05）.（2）The total effective rate of observation group was significantly higher than that of control group（P0.05）.（3）There was no significantly difference between two groups in the total incidence rate of adverse reactions（P0.05）.Conclusion NB-UVB combined with mucopolysaccharide polysulfate cream is effective in the treatment of psoriasis,and it can reduce IL-37,IFN-γand TNF-αlevels and relieve the symptoms of skin lesions with high safety.
