OICU医护合作访视在控制骨科围手术期高血压患者中的应用效果

摘　　要：目的探讨骨科重症监护病区(Orthopedics Intensive Care Unit,OICU)医护合作访视在控制骨科围手术期高血压患者中的应用效果。方法收集2014-01/2016-12月广州总医院OICU收治的447例骨科疾病合并高血压患者,按有无进行OICU医护联合围手术期访视分为对照组(n=158)和观察组(n=289),对照组进行常规术前访视,观察组在常规术前访视的基础上进行OICU医护联合围手术期访视,比较两组患者术前术后血压变化幅度、降血压医嘱合格例数、住院日及住院费用。结果观察组降血压医嘱合格率高于对照组,术后血压变化幅度、住院天数、住院费用均低于对照组,组间比较差异均有统计学意义(P<0.05)。结论 OICU医护合作访视可以提高医护人员对骨科围手术期高血压患者的血压控制,缩短患者的住院时间,减少住院费用,值得推广。Objective To explore the application effects of orthopedics intensive care unit（OICU）in controlling patients with high blood pressure in perioperative period.Methods A total of 447 orthopaedic patients with hypertension from OICU in Guangzhou General Hospital from January 2014 to December 2016 were collected.According to whether or not combination of OICU doctor-nurse cooperation and perioperative visiting,patients were divided randomly into control group with routine preoperative visiting（n=158）and observation group with OICU doctor-nurse cooperation plus routine preoperative visiting（n=289）.Changes of pre-and post-operative blood pressure,qualified cases of doctor＇s advice of blood pressure control,hospital stays and hospitalization expenses were compared between two groups.Results The qualified rate of doctor＇s advice of hypotensive in the observation group was significantly higher than that in the control group,and the postoperative blood pressure changes,hospital stays and hospitalization expenses in the observation group were remarkably less than those in the control group（P0.05）.Conclusion OICU doctor-nurse cooperative visiting can improve the control of perioperative blood pressure,shorten the hospital stays,reduce the hospitalization expenses.It is worth of being generalized in clinical.
