摘 要:目的在异位妊娠的超声检查过程中观察子宫螺旋动脉阻力指数(RI)和搏动指数(PI)等血流动力学的改变,评价子宫螺旋动脉阻力指数和搏动指数在异位妊娠超声诊断中的价值。方法选取2010年1月—2016年12月在我院妇产科确诊的宫腔内妊娠妇女100例和异位妊娠妇女100例作为研究对象。对所有研究对象均进行超声检查。上述对象在接受彩色多普勒超声检查过程中,测量子宫螺旋动脉阻力指数和搏动指数,分析子宫螺旋动脉阻力指数和搏动指数在异位妊娠超声诊断中的实用价值。结果宫腔内妊娠妇女子宫螺旋动脉阻力指数和搏动指数均低于异位妊娠妇女,差异具有统计学意义(P<0.05);经腹壁彩色多普勒超声、经阴道彩色多普勒超声以及彩色多普勒超声检查时测量子宫螺旋动脉阻力指数和搏动指数对异位妊娠的检出率对比,差异具有统计学意义(P<0.05)。结论彩色多普勒超声检查时测量子宫螺旋动脉阻力指数和搏动指数有助于提高异位妊娠的超声诊断准确率。Objective To observe the hemodynamic changes in the resistance index and pulsatility index of uterine artery in the diagnosis of ectopic pregnancy, to assess the value of resistance index and pulsatility index of uterine artery in the ultrasonic diagnosis of ectopic pregnancy. Methods 100 cases of intrauterine pregnancy women and 100 cases of ectopic pregnancy women in our hospital l^om January 2010 to December 2016 were selected as the research objects. All the subjects were examined by ultrasonography. During the examination of color Doppler ultrasonography, the resistance index and pulsatility index of uterine spiral artery were measured, and the value of uterine spiral artery resistance index and pulsatility index in the diagnosis of ectopic pregnancy by ultrasonography was analyzed. Results The resistance index and pulsatility index of uterine spiral arteries in intrauterine pregnancy women were lower than those in ectopic pregnancy women, and the difference was statistically significant (P 〈 0.05). In abdominal wall color Doppler ultrasound, transvaginal color Doppler ultrasound and color Doppler ultrasound examination, the difference of ectopic pregnancy detection rate of uterine spiral artery resistance index and pulsatility index was statistically significant (P 〈 0.05). Conclusion The measurement of resistance index and pulsatility index of uterine artery during ultrasonic examination could improve the diagnosis accuracy ofectopic pregnancy.
