温阳化癥膏联合盐酸羟考酮缓释片治疗癌性疼痛的临床观察

摘　　要：目的:观察温阳化癥膏联合盐酸羟考酮缓释片治疗癌性疼痛(痰瘀交阻)的临床疗效。方法:将160例患者随机分为治疗组(n=100)与对照组(n=60)。对照组给予盐酸羟考酮缓释片口服,10 mg为起始剂量,每12小时1次;治疗组在对照组治疗基础上予以温阳化癥膏外贴,每天1贴,每贴6~8 h,7 d为一疗程。治疗期间两组病例均可对等使用支持治疗,伴爆发痛时及时使用短效速释药物治疗,并依据滴定剂量调整盐酸羟考酮缓释片服用量。结果:总有效率治疗组为98.9%,对照组为95%,两组比较差异无统计学意义(P>0.05)。治疗组爆发痛次数、止痛药使用剂量明显少于对照组,两组比较差异有统计学意义(P<0.05)。结论:温阳化癥膏治疗癌性疼痛的止痛效果满意,患者生活质量得到明显改善,其治疗安全有效,毒副作用小,且简单易行,为中西医结合治疗癌性疼痛提供了新途径。Objective: To observe the clinical effect of Wenyanghuazheng Gao combined with oxycodone hydrochloride controlled-release tablets in the treatment of cancer pain( phlegm and blood stasis resistance). Methods:One hundred sixty cases of cancer were randomly divided into treatment group( n = 100) and control group( n =60),the control group was taken orally oxycodone hydrochloride prolonged-release tablets,10 mg as starting dose,12 h once; On this basis the treatment group was given the Wenyanghuazheng Gao,every day a paste of 6-8 h,7 d for a course of treatment. During the treatment,the two groups could be used peer support therapy,short-acting and quick-releaseing drug was timely used to deal with outbreaks of pain. According to the titration dose,oxycodone hydrochloride prolonged-release tablets dose was adjusted. Results: The total effective rate in the treatment groups was 98. 9% and 95% in the the control group. There was no significant difference between them( P > 0. 05).Frequency of outbreaks of pain and dosage of painkiller of the treatment group were less than that of the control group,there were significant difference( P < 0. 05). Conclusion: Wenyanghuazheng Gao has great analgesic effect for patients with cancer pain. It has a certain extent to improve the quality of life. It is safe and effective,non-toxic side,and simple to use. It provides a new treatment approach for cancer pain in combination of traditional Chinese and western medicine treatment.
