升降散治疗瘀毒内阻型脓毒症的临床研究

摘　　要：目的:观察升降散治疗瘀毒内阻型脓毒症的临床疗效。方法:将50例患者随机分为对照组和观察组,每组各25例;对照组给予西医常规治疗,观察组在西医常规治疗的基础上加用升降散,疗程7天。观察2组治疗前后慢性健康状况评分(APACHEⅡ)、C反应蛋白(CRP)、血小板计数(PLT)、肿瘤坏死因子α(TNF-α)、中医证候积分的变化情况和临床疗效。结果:治疗后2组APACHEⅡ评分、CRP、TNF-α均降低,PLT升高,且观察组优于对照组(P<0.05)。2组中医证候积分均降低,观察组疗效优于对照组(P<0.05)。结论:升降散结合西医治疗脓毒症风温病瘀毒内阻证的临床疗效优于单纯西医治疗,其机制可能与降低炎症因子、减轻炎症反应,同时调节血小板有关。Objective: To observe clinical effects of Sheng Jiang powder in treating sepsis of stasis-toxin internal obstruction pattern. Methods: All 50 patients were randomly allocated to the control group and the observation group, 25 cases each group, the control group accepted routine treatment of western medicine, and the observation group took Sheng Jiang powder on the foundation of the routine therapy of western medicine, the course was seven days. Clinical effects, the changes of TCM syndrome scales, TNF-α, PLT, CRP and the scales of APACHEⅡ of both groups were observed before and after treating. Results: The scales of APACHEⅡ, CRP and TNF-α of both groups lowered after treating, PLT raised, and the observation group was superior to the control group(P <0.05).TCM syndrome scales of both groups decreased, the observation group was superior to the control group in clinical effects(P<0.05). Conclusion: Clinical effects of Sheng Jiang powder combined with western medicine are superior to only western medicine in the treatment of sepsis of stasis-toxin internal obstruction pattern, and its mechanism might be related to reducing inflammatory factors, relieving inflammatory reaction and regulating blood platelet simultaneously.
