Shikani喉镜和Clarus可视喉镜在颈椎制动患者气管插管中的应用比较

摘　　要：目的比较Shikani可视喉镜和Clarus可视喉镜(Trachway)在颈椎制动患者气管插管中的应用效果。方法选择ASAⅠ~Ⅱ级,拟全身麻醉下行颈椎内固定术的患者60例。按随机数字表法分为Shikani可视喉镜(S组)和Clarus可视喉镜组(C组),每组30例。记录麻醉诱导前(T1)、麻醉诱导后插管前(T2)、插管后即刻(T3)、插管后1 min(T4)、3 min(T5)、5 min(T6)时刻的平均动脉压(MAP)、心率(HR)、心率-收缩压乘积(RPP)的情况,记录插管时间、1次插管成功率、插管次数、咽喉疼痛及相关并发症发生情况。结果 S组一次气管插管完成时间明显短于C组(P<0.05),轻度咽喉疼痛发生率及插管咽喉损伤率S组小于C组(P<0.05)。与T1比较,两组T2~T6时MAP及RPP降低(P<0.05),各时间点MAP、HR、RPP组间比较差异没有统计学意义(P>0.05)。首次插管成功率、插管次数两组比较差异没有统计学意义(P>0.05)。结论在颈椎制动患者气管插管中,与Clarus可视喉镜(Trachway)比较,Shikani喉镜插管时间缩短,但在插管时对血流动力学影响、首次插管成功率、插管次数及咽喉疼痛等相关并发症发生率等方面无明显差异。Objective To compare the application of Shikani optical stylet （SOS） and Clarus Video Stylet （Tracway） in patient with cervical spine immobilization in tracheal intubation. Methods Sixty patients, ASA I II , undergoing cervical internal fixation operation, were randomly divided into Shikani optical stylet group （Group S, n = 30） and Clams Video Stylet group （Group C, n = 30）. MAP, HR and RPP （the rate-pressure product） were recorded at the point before induction of anesthesia （T1）, before intubation （T2）, at the immediate time of intubation （T3） and 1 min （T4）, 3 rains （T5）, and 5 mins after intubation （T6）. The intubation time, one- time success rate of intuhatiun, the number of intubation times and the incidence of sore throat and other complica- tions were observed. Results The one-time intubation time in group S was obviously shorter than that in group C （P 〈 0.05）. The incidence of mild sore throat and intubation throat injury rate were lower in group S than those in group C （P 〈 0.05）. Compared with those at T1, MAP and RPP decreased significantly at point of T2 - T6 in both of two groups （P 〈 0.05）. There was no significantly difference in MAP, HR and RPP at any points of time between the two groups. The one-time success rate of intubation, the number of intubation times and the incidence of airway complications in two groups （P 〉 0.05） were no significantly different. Conelusions Compared with Clams Video Stylet （Tracway）, Shikani optical stylet can shorten the intubation time in patients with cervical spine immobilization, but no difference was found in regard to the hemodynamic influence on intubation, success rate of intuba- tion, the intubation times, the sore throat and the other related complications.
