丙泊酚吸入全身麻醉对老年手术患者术后认知功能的影响

摘　　要：目的观察和探讨丙泊酚吸入全身麻醉对老年手术患者术后认知功能的影响。方法选取2015年8月~2016年12月我院收治的78例老年手术麻醉患者,随机分为观察组和对照组,对照组吸入异氟醚1.0%~2.5%,静脉注射芬太尼1.0μg/(kg·h)维持麻醉;观察组以注射泵输注丙泊酚4.0~6.0mg/(kg·h)和瑞芬太尼1.0μg/kg。观察两组患者术后自主呼吸恢复时间、睁眼时间、定向力恢复时间,麻醉后6、12、24h简易智能量表(MMSE)评分的变化。结果与对照组苏醒质量比较,观察组术后苏醒质量有明显改善,差异有统计学意义(P<0.05)。与对照组比较,观察组术后6、12h的MMSE评分明显上升,差异有统计学意义(P<0.05),术后24h两组患者的MMSE评分差异无统计学意义(P>0.05)。结论丙泊酚吸入全身麻醉后苏醒质量明显增强,术后认知功能恢复快,适合于老年患者,值得推广使用。Objective To observe and explore propofol in general anesthesia on postoperative cognitive function in elderly patients with surgery.Methods From August 2015 to December 2016 78 cases in anesthesia department of our hospital were randomly divided into observation group and control group,in the control group with inhaling isoflurane 1.0%~2.5%,intravenous fentanyl 1.0μg/（kg·h）maintain anesthesia;the observation group with injection pump infusion of propofol 4.0~6.0 mg/（kg·h）and fentanyl 1.0μg/kg.The postoperative spontaneous breathing recovery time,eye open time,recovery time of directional force and 6 hours,12 hours,24 hours after anesthesia simple intelligence scale（MMSE）score changes in two group were observed.Results Awakening quality after waking up in control group improved more significantly than that in observation group,with significant difference（P〈0.05）.MMSE score after 6 hours,12 hours in the control group increased more dramatically than observation group,with significant difference（P〈0.05）,the MMSE score postoperative 24 hours in two groups was no significant difference（P〉0.05）.Conclusion Propofol can significantly enhance quality after general anesthesia,fasten postoperative recovery of cognitive function,be suitable for elderly patients,and be worthy of being promoted.
