摘 要:目的探讨超声弹性成像(ultrasoundelastography,UE)联合甲状腺影像学报告及数据系统(thyroidimagingreportinganddatasystem,TI—RADS)对桥本甲状腺炎(Hashimotothyroidifis,HT)背景下伴发结节良恶性质进行判定的应用价值。方法选取2016年3月至2017年4月因HT伴发结节患者93例(159个结节),先应用常规超声对结节进行检查并作TI—RADS分类,后应用UE对结节进行检查并作UE评分,并与术后病理结果进行比较分析。结果UE评分诊断恶性HT伴发结节的灵敏度(71.43%)、准确率(77.36%)分别〈TI-RADS分类的灵敏度(84.52%)、准确率(86.79%)分别〈两者联合的灵敏度(95.24%)、准确率(94.34%),各方法间比较差异均有统计学意义(均P〈O.05)。结论UE联合TI—RADS分类共同对HT伴发结节进行良恶性诊断的效能优于单一方法,能进一步提高灵敏度及准确率,值得推广应用。Objective To investigate the application value of diagnosis of benign and malignant nodules with Hashimoto thyroiditis ( HT ) by ultrasonic elastography ( UE ) combined with thyroid imaging reporting and data system ( TI-RADS ) . Methods The patients with nodules with HT as the research objects in our hospital from March 2016-April 2017, including a total of 93 cases ( 159 nodules ) . First, the nodules were examined by conventional ultrasound for TI-R.ADS classification, then examined by UE for HE score, and compared with the pathological results after operation. Results The sensitivity (71.43%) , accuracy (77.36%) of UE score in diagnosis of malignant nodules with HT, respectively, 〈the sensitivity ( 84.52% ) , accuracy ( 86.79% ) of TI-RADS classification, respectively, 〈the sensitivity ( 95.24% ) , accuracy ( 94.34% ) of UE combined with TI-tLADS, the differences were statistically significant ( all P〈0.05 ) . Conclusions UE combined with TI-RADS is superior to single method in the diagnosis of benign and malignant nodules with HT. It can further improve the sensitivity and accuracy, and is worthy of popularization and apphcation.
