超声引导腹横肌平面阻滞应用在下腹部手术后的镇痛效果

摘　　要：目的探究在超声引导下的腹横肌平面阻滞(TAP)应用于下腹部手术患者术后镇痛的效果。方法选择我院2016年4月—2017年2月收治的全麻下行下腹部手术患者200例,随机数进行分组,每组100例,实验组患者注射0.2%罗哌卡因20 ml,而对照组患者注射等量生理盐水,分析两组患者镇痛效果。结果实验组与对照组之间VAS以及Ramsay评分差异具有统计学意义(P<0.05)。24 h内实验组患者VAS评分有明显下降趋势,两组患者均未发生不良反应。结论将超声引导应用于TAP阻滞能够有效满足下腹部手术后患者镇痛要求,能够起到缓解患者疼痛的效果。Objective To explore the postoperative analgesic effect of ultrasound-guided transversus abdominis plane block（TAP） in lower- abdominal surgeries. Methods Two hundred patients who were admitted to our hospital from April 2016 and February 2017 and underwent lower-abdominal surgery under general anesthesia were randomly assigned to the test group or control group each including 100 patients. Patients in the test group received injection of 0.2% ropivacaine 20 ml, and patients in the control group received injection of equal volume of normal saline. The analgesic effect was analyzed for both groups. Results There were significant differences in VAS and Ramsay scores between the experimental group and the control group （P 〈 0.05）. Over 24 h, there were significant decreases in VAS score in the test group, and no adverse reactions were observed in either group. Conclusion Ultrasound guided TAP block can effectively meet the analgesic requirements of patients after lower abdominal surgery, and can relieve the pain of patients.
