转化生长因子β1和结缔组织生长因子与高血压左心室肥厚的相关性

摘　　要：目的研究结缔组织生长因子(Connective tissue growth factor,CTGF)和转化生长因子β1(Transforming growth factorβ1,TGF-β1)水平与原发性高血压左心室肥厚(Left ventricle hypertrophy,LVH)的相关性。方法选择2015年2月—2016年7月收治的原发性高血压患者118例,将其分为LVH组62例和未合并LVH组(NLVH组)56例,另外选择30例健康体检者作为对照组。采用酶联免疫法检测血清TGF-β1和CTGF水平,采用彩色超声多普勒检测各组左室舒张末期内径、左室后壁厚度、心脏舒张期室间隔厚度及左室射血分数,计算左心室质量(Left ventricular mass,LVM)和左室质量指数(LVMI)。计量资料比较采用单因素方差分析或LSD-t检验,两变量间相关关系采用Pearson相关分析,多重因素分析采用多元线性逐步回归分析。P<0.05为差异有统计学意义。结果 LVH组和NLVH组收缩压、舒张压、体重指数、TC、LDL-C高于对照组,差异有统计学意义(均P<0.05)。NLVH组TGF-β1、CTGF及LVMI高于对照组,且LVH组高于对照组及NLVH组,差异有统计学意义(均P<0.05)。相关分析显示TGF-β1和CTGF呈正相关,且其数值越大,相关性越强。结论高血压左心室肥厚患者血清中TGF-β1、CTGF、LVMI水平均较高,且SBP、TC是影响高血压左室肥厚患者TGF-β1与CTGF水平的相关因素,共同参与左心室肥厚的病理生理过程。Objective To study the correlation between serum levels of transforming growth factor-β1（TGF-β1）and connective tissue growth factor（CTGF）with hypertensive left ventricle hypertrophy（LVH）.Methods A total of 148 patients with hypertension were divided into two groups,62 cases in LVH group and 56 cases in without LVH group（NLVH group）.And 30 healthy persons were selected as control group.Serum levels of TGF-β1 and CTGF were detected by enzyme-linked immunosorbent assay.Left ventricular end-diastolic diameter,posterior wall thickness of left ventricle,interventricular septum thickness and left ventricular ejection fraction were measured by color Doppler sonography.Left ventricular mass（LVM） and Left ventricular mass index（LVMI） were calculated.Measurement data uses one-way ANOVA or LSD-t test.The correlation between the two variables was analyzed by Pearson correlation analysis,and multiple linear regression analysis was used for multiple factor analysis.P〈0.05 was considered statistically significant.Results The systolic blood pressure,diastolic blood pressure,BMI,TC and LDL-C in the LVH group and NLVH group were significantly higher than those in the control group（all P〈0.05）.The levels of TGF-β1,CTGF and LVMI in the NLVH group were higher than those in the control group,and those in the LVH group were higher than control group and NLVH group.The difference was statistically significant（all P〈0.05）.Correlation analysis showed that TGF-β1 and CTGF were positively correlated,and the greater the value,the stronger the correlation.Conclusion Serum levels of TGF-β1,CTGF and LVMI were significantly elevated in hypertensive patients with left ventricular hypertrophy.Systolic blood pressure and TC were the related factors of TGF-β1 and CTGF in hypertensive left ventride hypertrophypatients,and participate in the pathophysiological process of left ventricular hypertrophy.
