淮安市2型糖尿病合并高血压患者血压控制情况及其影响因素分析

摘　　要：目的了解淮安市2型糖尿病合并高血压患者的血压控制情况及影响因素,为制订糖尿病合并高血压的防治措施提供依据。方法采用分层整群抽样法抽取2013年12月31日前在社区卫生服务中心登记管理的糖尿病合并高血压患者6 941例,采用统一问卷询问其人口学资料、生活方式及行为、疾病史和治疗史等,测量身高、体重和血压,进一步检测其血糖和血脂。采用SPSS 19.0统计软件进行血压控制影响因素的单因素和多因素logistic回归分析。结果淮安市糖尿病合并高血压患者中,血压控制良好率为13.20%。70岁及以上(OR=0.70,95%CI:0.51~0.98)、吸烟(OR=0.72,95%CI:0.60~0.86)、肥胖(OR=0.76,95%CI:0.59~0.97)、中心性肥胖(OR=0.79,95%CI:0.66~0.94)、高胆固醇血症(OR=0.75,95%CI:0.58~0.97)和高甘油三酯血症(OR=0.78,95%CI:0.66~0.93)是血压控制的危险因素(P<0.05,P<0.01),而居住在城市(OR=2.03,95%CI:1.71~2.42)、大专及以上学历(OR=2.14,95%CI:1.45~3.16)、进行降压治疗(OR=8.59,95%CI:7.56~9.77)和血糖控制良好(OR=1.35,95%CI:1.16~1.57)是血压控制的保护因素(P<0.01)。结论淮安市糖尿病合并高血压患者的血压控制率很低,应加强患者血压、血糖、血脂管理及吸烟和肥胖等其他危险因素控制。Objective To understand the control rate and influencing factors of hypertension in type 2 diabetic patients of Huai ＇an city, and to provide the basis for preventing and treating the diabetes with hypertension. Methods The stratified cluster random sampling method was used to select 6 941 type 2 diabetic patients with hypertension from the community health service centers as the subjects. The investigation was performed with the questionnaire and physical examination. The questionnaire contained the demographic characteristics, life style and behavior, history of diseases and history of treatment. The physical examination included the height, weight, blood pressure, blood glucose and blood lipid. Single factor and multi factor logistic regression methods were utilized to analyze the influencing factors of hypertension. Results In diabetic patients with hypertension, the good control rate was13.20%. The risk factors of hypertension control were age（≥70 years old： OR=0.70, 95%CI： 0.51-0.98）, smoking（OR=0.72,95%CI： 0.60-0.86）, obesity（OR=0.76, 95%CI： 0.59-0.97）, central obesity（OR=0.79, 95%CI： 0.66-0.94）, high total cholesterol（OR=0.75, 95%CI： 0.58-0.97） and high triacylglycerol（OR=0.78, 95%CI： 0.66-0.93）, P0.05 or P0.01. The protection factors of hypertension control were urban residence（OR=2.03, 95%CI： 1.71-2.42）, ≥college education level（OR=2.14, 95%CI： 1.45-3.16）, antihypertensive treatment（OR=8.59, 95%CI： 7.56-9.77） and good glycemic control（OR=1.35, 95%CI： 1.16-1.57）, P0.01. Conclusion The control rate of blood pressure in diabetic patients with hypertension of Huai ＇an is very low, the control measures of hypertension, blood glucose, blood lipid, smoking and obesity should be taken.
