温和悬灸治疗椎管内麻醉术后尿潴留疗效观察

摘　　要：目的探讨椎管内麻醉术后发生尿潴留患者应用温和悬灸干预后恢复排尿的作用机制,探讨其临床疗效及护理价值,为椎管内麻醉术后尿潴留的治疗提供一种安全、有效、简便的方法。方法选取我院椎管内麻醉术后尿潴留患者91例,随机分为对照组和观察组。对照组给予肌肉注射用甲硫酸新斯的明药液2ml,观察组术后给予温和悬灸治疗。两组经过上述相应处理,如果12h后仍不能自行排尿者,可采取无菌导尿术缓解症状。观察并分析两组患者临床疗效、第一次排尿时间、以及对生活质量评分的影响。结果治疗后对照组总有效率为76.74%,观察组总有效率为91.67%,两组临床疗效比较,观察组优于对照组,差异有统计学意义(P<0.05);第一次排尿时间,观察组短于对照组,治疗后观察组生活质量评分高于对照组,差异有统计学意义(P<0.05)。结论温和悬灸作为一种传统的中医理疗方法,在治疗椎管内麻醉术后尿潴留方面疗效明显,可有效缩短排尿时间,减少相应的导尿操作,改善患者生活质量,利于病情恢复。Objective To investigate the mechanism of spinal anesthesia for postoperative urinary retention in patients treated with mild moxibustion intervention restored micturition, and to investigate its clinical effect and value of nursing care and provide a safe and effective method for the treatment of intraspinal anesthesia urinary retention.Methods Totally 91 cases of urinary retention after spinal anesthesia in author＇ s hospital were randomly divided into thecontrol group and the observation group.The observation group was given mild moxibustion treatment; the control group was given the medicine 2mL intramuscular injection of neostigmine. Two groups after the above treatment, 12h symptoms still cannot urinate voluntarily, feasible aseptic catheterization.After treatment, the clinical efficacy, first times of urination, the quality of life score were observed and analyzed in the two groups.Results The clinical efficacy of the two groups after treatment, control group total effective rate was 76.74%.It was 91.67% in the observation group.So the Observation group was better than the control group.In the first micturition time,the observation group was shorter than the control group,the observation group after treatment quality of life score higher than the control group. All of above have the difference in statistically significant （P〈0.05）.Conclusion The application of mild moxibustion treatment of intraspinal anesthesia postoperative urinary retention curative effect significantly, can effectively shorten the time of urination, reduce the catheterization operation, improve the quality of life of patients,enhance the expression of HSP,and it is conducive to disease recovery.
