摘 要:目的探究呼出气一氧化氮(FeNO)水平与结缔组织病相关肺间质病变(CTD-ILD)的相关性。方法选取2015年1月~2016年12月承德医学院附属医院收治的34例CTD-ILD患者,设为CTD-ILD组;另选取42例单纯肺间质病变(ILD)患者,设为ILD组;选取同期健康体检者40例,设为对照组。对比3组受检者的肺功能及FeNO水平差异,并对FeNO水平与血沉(ESR)、C反应蛋白(CRP)、类风湿因子(RF)、抗核抗体(ANA)、第1秒用力呼气量占肺活量比值(FEV1/FVC)的相关性进行统计分析。结果 CTD-ILD组与ILD组的最大呼气流量占比(PEF)、1秒用力呼气末容积占比(FEV1)、用力肺活量占比(FVC)相比,差异均无统计学意义(P>0.05),但CTD-ILD组和ILD组的肺功能指标PEF、FEV1、FVC、FeNO与对照组相比差异均有统计学意义(P<0.05);3组FeNO水平对比显示,CTD-ILD组>ILD组>对照组,各组间差异均有统计学意义(P<0.05)。FeNO水平与CRP呈显著正性相关(P<0.05)。结论 CTD-ILD患者的FeNO水平有明显增高趋势,与机体的炎症反应及肺功能显著相关,FeNO测定结合肺功能检查对于CTD-ILD诊断,提供了新的思路。Objective To investigate the correlation between exhaled nitric oxide (FeNO)and connective tissue disease- related pulmonary interstitial lesions (CTD-ILD). Methods Thirty-four patients with CTD-ILD were enrolled in this study. The 34 patients were treated for CTD-ILD and 42 patients for pulmonary interstitial disease(ILD). The ILD group and healthy (control)group were measured for rheumatoid factor(RF),antinuclear antibody(ANA)and C-reactive protein(CRP),and the changes of lung function and FeNO level were compared with those of the three groups. One-second rate (FEV1/FVC)correlation analysis was performed. Results There was no significant difference between the CTD-ILD group and the ILl) group (PEF), l-second forced expiratory volume (FEV1)or forced vital capacity (FVC)(P 〈 0.05). Compared with the control group,the pulmonary function indexes of the CTD-ILD group and the ILD group were significantly different from those of the control group (P 〈 0.05). The comparison of FeNO levels showed that CTD-ILD group 〉 ILD group 〉 control group (P 〈 0.05). The level of FeNO positively correlated with CRP levels (P 〈 0.05). Conclusion The level of FeNO in CTD-ILD patients is significantly higher than that in patients with CTD-ILD,which is related to the inflammatory response and pulmonary function of CTD-ILD. As such,the FeNO levels can be used as a diagnostic index of CTD-ILD.
