益骨汤对绝经后骨质疏松患者雌激素水平及情志的影响

摘　　要：目的:探讨益骨汤对绝经后骨质疏松患者雌激素水平及情志的影响。方法:选择120例确诊绝经后骨质疏松症患者随机分为观察组和对照组各60例。对照组予以双膦酸盐+基础治疗(钙剂+维生素D),观察组在对照组治疗基础上联合应用中药益骨汤加减治疗。于治疗前、治疗6个月后,分别观察2组患者的临床症状、情绪变化,检测腰椎骨密度以及血清E_2、TRAP5b水平。结果:观察组总有效率78.3%,对照组53.3%,观察组症状改善优于对照组(P=0.015);治疗前后两组骨密度相比较,观察组骨密度改善显著(P=0.000),对照组无明显差异(P=0.060),观察组骨密度明显高于对照组(P=0.000);治疗后两组情志状态好转明显,HAMA量表评分显著降低(P=0.000,P=0.044),观察组显著低于对照组(P=0.001);治疗后观察组雌激素水平明显高于对照组(P=0.020),治疗后观察组雌激素水平均显著高于治疗前(P=0.000);观察组TRAP5b水平明显低于对照组(P=0.001);治疗后观察组TRAP5b水平均显著低于治疗前(P=0.006)。结论:对绝经后骨质疏松患者在常规抗骨质疏松治疗的同时,联合应用益骨汤治疗能有效改善骨密度,提高雌激素水平,改善患者的情志状态。Objective：To explore the effects of Yigu Tang on serum E_2 level and anxiety of women with postmenopausal osteoporosis.Methods：120 Cases with postmenopausal osteoporosis were divided into observation and control groups at random with each 60 cases,the control group patients were given diphosphonate and basic treatment,the observation group patients were given Yigu Tang on the basis of control group treatment,they were treated for 6 months,the changes of clinical symptoms and anxiety in patients were observed,bone density,serum E_2 and TRAP5b levels were determined before and after treatment.Results：The total effective rate of observation was 78.3%,that was better than that of control,that was 53.3%（P = 0.015）.The bone density of observation group patients was improved significantly after treated,that was more significant than that of control group which was no obvious change（P = 0.00）.Anxiety state of the two group women were alleviated,HAMA scale score was lowered significantly,the effect of observation group was more obvious than that of control（P =0.001）.Serum E_2 level in the two groups was raised and TRAP5b was decreased after treated to compare with pro-treatment,the effects of observation group were better than those in control group（P = 0.02;P = 0.001）.Conclusion：Yigu Tang can effectively strengthen bone density of postmenopausal osteoporosis women,increase serum estrogen level and improve anxiety.
