踝、趾臂指数与脉搏波速度在老年2型糖尿病合并下肢动脉病变患者诊断中的价值

摘　　要：探讨老年2型糖尿病患者下肢动脉病变与踝臂指数(ABI)、趾臂指数(TBI)与肱踝脉搏波速度(baPWV)的相关性。选取老年2型糖尿病患者101例,糖尿病合并高血压患者100例,另选同期体检的健康者100例作为对照组,所有患者均进行ABI、TBI、baPWV及下肢动脉血管超声检查对照组、糖尿病组、糖尿病合并高血压组三组间ABI、TBI及baPWV差异有统计学意义(P<0.05)。以血管超声为金标准,ABI诊断血管狭窄的敏感性为76.34%,特异性为91.35%,TBI与ABI有较好的一致性,下肢动脉狭窄程度与ABI、TBI呈负相关(P<0.05)。ABI、TBI及baPWV与老年2型糖尿病显著相关,在老年2型糖尿病及合并高血压患者有较大应用价值。ABI、TBI检查与下肢血管超声相关性好,且其操作简便,无创伤性,宜在临床上推广应用。ABI、TBI及baPWV对早期发现老年2型糖尿病患者下肢动脉粥样硬化具有重要的临床意义。To explore the correlation between brachial-ankle pulse wave velocity（baPWV）,ankle-brachial index（ABI）,toebrachial index（TBI）and lower limbs arterial disease in elder patients with type 2 diabetes mellitus（T2 DM）,providing clinical evidence for early prevention of lower limbs atherosclerosis.101 patients with type 2 Diabetes Mellitus,100 patients with T2 DM and hypertension（HP）and 100 health persons as control group were recruited from outpatient of endocrinology departments in the Second Affiliated Hospital of Dalian Medical University.Variance of ABI,TBI and baPWV were analyzed between the T2 DM group,the T2 DM hypertension group and the control group.ABI,TBI and baPWV were evaluated via the associations with the severity of the lower limbs arterial lesions.ABI,TBI and baPWV of patients in the three groups were significantly changed（P〈0.05）.Considering lower limbs artery blood vessels ultrasound examination as the golden standard,we observed that the specificity and sensitivity of ABI examination was 76.34% and91.35%,respectively.The values of ABI,TBI and baPWV were negative correlated to the status of lower limbs atherosclerosis（P〈0.05）.Results from ABI,TBI and baPWV examination are significantly correlated with those from lower limbs arteries ultrasound examination.ABI,TBI and baPWV are related to the T2 DM.As a non-invasive technolog,ABI,TBI and baPWV examinations process simple and convenient operation and worthy to be popularized for its simplicity.ABI,TBI and baPWV examination provides an insightful approach for early diagnosis of lower limbs arterial lesions in patients with T2 MD.
