小剂量轻比重0.5%布比卡因腰-硬联合麻醉在高龄下肢手术患者中的应用效果

摘　　要：目的探讨小剂量不同比重布比卡因腰-硬联合麻醉在高龄下肢手术患者中的应用效果。方法选取泰和县中医院2016年4月—2017年3月收治的下肢手术高龄患者120例,根据麻醉药物不同比重分为观察组和对照组,各60例。观察组采用小剂量轻比重0.5%布比卡因腰-硬联合麻醉,对照组采用等比重0.5%布比卡因腰-硬联合麻醉。比较两组患者的麻醉效果及血压、心率、血氧饱和度。结果两组患者麻醉效果比较,差异无统计学意义(P>0.05)。麻醉前,两组患者血压、心率及血氧饱和度比较,差异无统计学意义(P>0.05);麻醉后,观察组患者各指标较对照组平稳(P<0.05)。结论小剂量轻比重0.5%布比卡因腰-硬联合麻醉在高龄下肢手术患者中的应用效果好,且患者各项生命体征更加平稳,麻醉安全性高。Objective To discess the combined spinal epidural anesthesia with low dose light weight 0. 5% bupivacaine in lower extremity surgery in elderly patients. Methods A total of 120 cases of lower extremity surgery in elderly patients were selected from April 2016 to March 2017 in Taihe Chinese Medicine Hospital,which were divided into observation group and control group according to different weights. Observation group was given combined spinal epidural anesthesia with low dose light weight 0. 5% bupivacaine,control group was given combined spinal epidural anesthesia with equal weight 0. 5% bupivacaine.The anesthesia effect,blood pressure,heart rate and blood oxygen saturation were compared between the two groups. Results No siginificat difference of anesthesia effect was found between the two groups（ P 〉0. 05）. Before anesthesia,no siginificat differences of blood pressure,heart rate or blood oxygen saturation was found between the two groups（ P 〉0. 05）; after anesthesia,the indexes of observation group was more stable than control group（ P 〈 0. 05）. Conclusion Combined spinal epidural anesthesia with low dose light weight 0. 5% bupivacaine has a better anesthesia effect in lower extremity surgery in elderly patients,and the vital signs of patients are more stable,anesthesia safety is high.
