缓释型促性腺激素释放激素拟似剂治疗性早熟的疗效观察

摘　　要：目的　探讨缓释型促性腺激素释放激素拟似剂醋酸亮丙瑞林治疗特发性中枢性性早熟 (ICPP)女童治疗前和治疗 6个月后的临床早熟表现的变化 ,以及血清雌二醇 (E2 )、胰岛素样生长因子Ⅰ (IGF Ⅰ )、胰岛素样生长因子结合蛋白 (IGFBP3 )浓度变化。方法　 2 7例ICPP女童均接受缓释型醋酸亮丙瑞林治疗 6个月 ,每公斤体重 5 0～ 10 0 μg ,每月皮下注射 1次。于治疗前至治疗 6个月末观察身高、体重、性征、骨龄、B超子宫、卵巢的变化 ,以及血清E2 、IGF Ⅰ、IGFBP3 浓度变化。结果　接受缓释型醋酸亮丙瑞林治疗的 2 7例ICPP女童 ,治疗 6个月末与治疗前相比 ,性征消退 ,生长速度下降 ,骨龄进展缓慢 ,子宫、卵巢、E2 恢复到青春期前水平 ,而血清IGF Ⅰ和IGFBP3 水平差异无显著意义 (P >0 0 5 )。结论　用缓释型促性腺激素释放激素拟似剂治疗ICPP女童 6个月后 ,临床早熟表现明显得到控制 ,血清E2 水平明显降低。血清IGF Ⅰ和IGF BP3 水平无明显改变。Objective To study the changes of clinical manifestations,serum levels of estradiol(E 2),insulin-like growth factor Ⅰ(IGF-Ⅰ),and insulin-like growth factor binding protein(IGFBP 3) in female children with idiopathic central pubertas praecox(ICPP) on before and after being treated for 6 months with leuprolide acetate,the slow-release analog of the gonadotropin releasing hormone(GnRH-a).Methods Twenty-seven female children with ICPP underwent a 6-month treatmnet with slow-release leuprolide acetate at the hypodermic injective dosage of 50～100 mg/kg(body weight) monthly.The body height,body weight,sexual character,skeletal age,B-type ultrasound configurations of ovaries and uterus were observed,and the serum level of E 2,IGF-Ⅰ and IGFBP 3 were monitored,before and at the end of the 6-month treatment.Results The retrogression of sexual character,reducing of growth speed,slow progression of skeletal age,uterus,ovary and serum E 2 level returning to the situation before pubertas praecox were observed in the 27 female children with ICPP at the end of the 6-month treatment with the slow-release leuprolide acetate,while the serum levels of IGF-Ⅰ and IGFBP 3 kept unchanged(P>0.05).Conclusion After 6 months of treatment with the slow-release analog of the gonadotropin releasing hormone,the clinical symptoms of female children with ICPP are markedly controlled and the serum level of E 2 decrease obviously,but the serum levels of ICF-Ⅰ and IGFBP 3 keep unchanged.
