摘  要:目的评定湖北省十堰市糖尿病的失能权重。方法对湖北省十堰市3家综合性三甲医院共120名患者进行调查,并通过德尔菲法专家赋值、患者EQ-VAS评分赋值及调查者赋值3种方式获取失能权重。最后分别采用秩和检验、Kruskal-Wallis检验比较变量之间的差异,多元逐步回归模型分析伤残权重的影响因素。结果患者自评VAS失能权重:年龄别失能权重为0.250~0.400,各年龄阶段失能权重差异有统计学意义(χ~2=7.658,P<0.05)。性别差异无统计学意义(χ~2=0.050,P>0.05),多元逐步回归分析显示年龄、并发症种类、医疗保健3个变量对失能权重影响有统计学意义(P<0.05)。调查者失能权重赋值:年龄别失能权重为0.094~0.325,不同年龄阶段差异有统计学意义(χ~2=4.953,P<0.05)。性别差异无统计学意义(χ~2=1.212,P>0.05)。多元逐步回归分析显示年龄因素对失能权重具有统计学意义(P<0.05)。通过专家赋值、患者自评VAS及调查者赋值获得的5种后遗症失能权重均明显高于GBD2015失能权重。结论失能权重的大小受年龄的影响,对湖北省十堰市糖尿病失能权重的评定使用的3种赋值方法结果均高于全球疾病负担研究数据。Objective The aim of this study was to evaluate the disability weights of diabetes in Shiyan Hubei province. Methods 120 diabetic patients were investigated in three comprehensive hospitals in Shiyan, and the disability weights of diabetes were obtained from the experts used Delphi method, from the patients using EQ-VAS and from the investigators. Rank sum test and Kruskal-Wallis test were used to examine the differences between disability weights. Multi-factor linear regression was used to analyze the risk factors of disability weight. Results For the disability weights from the EQ-VAS, age-specific weight ranged from 0.250 to 0.400. Significant differences were found among different age groups (~2=7.658, P〈0.05), but differences were not found between male and female (X^2=0.050, P〉0.05). Muhi-factor linear regression models indicated that the disability weight was significantly associated with age, sequelae, health care (P〈0.05). For the disability weights from investigators, age-specific weight ranged from 0.094 to 0.325. Significant differences were found among different age groups (X^2=-4.953, P〈0.05), but differences were not found between male and female (X^2=1.212, P〉0.05). Multi-factor linear regression models indicated that the disability weight was significantly associated with age (P〈0.05). The disability weights from these three methods were all higher than GBD. Conclusion Age is a very important factor affecting disability weight. These three different results of the disability weights of diabetes mellitus were higher than the data of GBD.
