复杂性区域疼痛综合征48例临床特征分析

摘　　要：目的分析复杂性区域疼痛综合征(CRPS)的临床特征、治疗方法及预后。方法回顾性分析2004年1月—2016年12月首都医科大学宣武医院住院的48例CRPS患者的临床特征、治疗方法及其预后。结果 48例患者中,男23例(47.9%),女25例(52.1%),发病年龄20~73(54±13)岁,确诊年龄30~73(57±10)岁。病程1个月~40年,中位病程35.3个月;CRPSⅠ型女性多见,病程长,肿胀及红细胞沉降率和CRP升高者多于CRPSⅡ型。外伤和手术后出现四肢及肢端局限性红肿、疼痛、麻木及肌肉萎缩的患者,均应该考虑CRPS的可能性;CRPSⅠ型患者采用内科治疗,CRPSⅡ型患者应用外科神经阻滞治疗。结论外伤和手术后出现四肢及肢端局限性红肿、疼痛、麻木及肌肉萎缩的患者,均应该考虑CRPS的可能性;CRPSⅠ型患者内科治疗疗效好,CRPSⅡ型患者应用外科神经阻滞疗效优于内科治疗。。Objective To analyze the clinical characteristics and efficacy of complex regional pain syndrome （CRPS）.Methods The clinical characteristics, treatment and prognosis of 48 patients with CRPS who were hospitalized in Xuanwu Hospital of Capital Medical University from January 2004 to December 2016 were analyzed retrospectively.Results In the 48 patients,23 were male（47.9%）and 25 were female（52.1%）.The age of onset was 20-73（54 ＆#177;13）years,and the age of diagnosis was 30-73（57 ＆#177;10）years.The course of disease ranged from 1 month to 40 years,and the median course of disease was 35.3 months.The female patients in CRPS type I group were more likely to have a longer course of disease,and the rates of swelling and erythrocyte sedimentation rate and CRP increased more than those in CRPS type II group.Limbs acral swelling,limitation of pain,numbness and muscle atrophy in patients with trauma and surgery,all should consider the possibili-ty of CRPS;the curative effect of CRPS type I patients with medical treatment showed good effect, CRPS patients with type II surgery nerve block is superior to medical therapy.Conclusion The possibility of CRPS may be considered in patients with lim-ited redness,swelling,pain,numbness and muscle atrophy after trauma and operation.The medical treatment of CRPS type I is effective,and the therapeutic effect of surgical nerve block is superior to that of medical treatment in patients with CRPS type II.
