摘 要:目的分析亚临床甲状腺功能减退症(SCH)患者血脂、同型半胱氨酸及血常规指标水平的变化,并探讨其与促甲状腺激素(TSH)水平的相关性。方法收集2017年1月1日至6月30Et在山东大学附属省立医院内分泌科就诊的249例SCH患者的病历资料(SCH组)。同期收集200例健康体检者的资料作为正常对照组。将所有受试者根据血清TSH水平分为4组:A组(0.55~1.89mIU/L,66例)、B组(1.90~4.78mlU/L,134例)、c组(4.79~10.00mIU/L,192例)、D组(〉10.00mIU/L,57例)。比较各组血清促甲状腺激素(TSH)、游离三碘甲腺原氨酸(盯,)、游离甲状腺素(FT。)、血脂、同型半胱氨酸(Hey)、血常规指标水平;分析TSH与这些指标的相关性。结果①SCH组TSH、总胆固醇、三酰甘油、低密度脂蛋白胆固醇(LDL-c)、小而密低密度脂蛋白胆固醇(sdLDL-c)、游离脂肪酸、脂蛋白a、Hcy、白细胞计数、红细胞体积分布宽度、单核细胞百分比和绝对值均明显高于正常对照组,高密度脂蛋白胆固醇(HDL—C)、红细胞计数、血红蛋白、红细胞比容均明显低于正常对照组(均P〈0.01)。②甲状腺功能正常的2组,即A组和B组间各指标比较差异均无统计学意义(均P〉0.05)。c组总胆固醇、三酰甘油、LDL-C、sdLDL-C、游离脂肪酸、脂蛋白a、Hey、单核细胞百分比和绝对值明显高于A组及B组,HDL-C、血红蛋白、红细胞比容明显低于A组及B组(P〈0.05或P〈0.01)。D组总胆固醇、三酰甘油、LDL-C、sdLDL-C、游离脂肪酸、脂蛋白a、Hey明显高于C组,HDL-C明显低于c组[(5.6±1.6)mmol/L比(5.1±1.2)mmol/L、(2.0±1.1)mmol/L比(1.6±0.9)mmoL/L、(3.4±1.0)mmO]J/L比(3.0±1.0)rnmoL/L、(1.06±0.46)mmoL/L比(0.99±0.37)nlmo]/L、(0.62±0.26)mmol/L比(0.59±0.30)mmObjective To investigate the relevance among subclinical hypothyroidism (SCH), thyroid stimulating hormone(TSH), blood lipid, homocysteine (Hcy) and blood routine indicators. Methods Clinical data of 249 SCH patients( SCH group) and 200 healthy people(normal control group) admitted from January 1st, 2017 to June 30th, 2017 in Shandong Provincial Hospital Affiliated to Shandong University were retrospectively analyzed. All subjects were divided into 4 groups by TSH level: group A (0. 55-1.89 mIU/L, 66 cases) , group B (1.90-4.78 mIU/L, 134 cases), group C (4.79-10.00 mIU/L, 192 cases), group D( 〉 10.00 mIU/L, 57 cases ). TSH, fi'ee triiodothyronine( FT3 ) , free thyroxine( FT4 ) , blood lipid, Hcy and blood routine indicators were analyzed. Results ①Levels of TSH, total cholesterol ( TC), triacylglycerol ( TG), low-density lipoprotein cholesterol(LDL-C), small and dense low-density lipoprotein cholesterol (sdLDL-C), nonesterified fatty acid (NEFA), lipoprotein a(Lpa), Hcy, white blood cell count, red blood cell distribution width, percentage and absolute value of monocyte in SCH group were significantly higher than those in normal control group; levels of high-density lipoprotein cholesterol( HDL-C), red blood cell count, hemoglobin and erythrocrit in SCH group were significantly lower than those in normal control group(P 〈 0. 01 ). ②Blood lipid, Hcy and blood routine indicators showed no significant differences between group A and B (P 〉 0. 05). TC, TG, LDL-C, sdLDL-C, NEFA, Lpa, Hcy, percentage and absolute value of monocyte in group C were significantly higher and HDL-C, hemoglobin and erythrocrit were significantly lower than those in group A and B ( P 〈 0. 05 or P 〈 0. 01 ).
