穴位贴敷联合常规抗结核治疗对复治性肺结核患者免疫功能的影响

摘　　要：目的:探讨中药穴位贴敷联合常规抗结核治疗对复治性肺结核患者免疫功能的影响。方法:选取2014年5月至2016年5月青岛市胸科医院收治的复治性肺结核患者54例,按照治疗方案分为对照组(n=26)和观察组(n=28)。对照组采用3HRZE/9HRE标准化疗方案,观察组联合中药穴位敷贴,4周为1个疗程,连续治疗2个疗程。统计并比较治疗后2组临床疗效;检测并比较2组肺功能、免疫功能相关指标变化;采用简明健康测量量表(SF-36)对2组患者的生命质量进行评估。结果:观察组病灶吸收率为75.00%,显著高于对照组的46.15%(P<0.05)。与治疗前比较,治疗后2组FEV1、FVC和FEV1/FVC均升高(P<0.05或P<0.01),且观察组显著高于对照组(P<0.01);治疗后观察组CD3^+、CD4^+及CD4^+/CD8^+水平较治疗前及对照组升高(P<0.05或P<0.01);CD8^+水平低于治疗前及对照组(P<0.01);免疫球蛋白Ig G、Ig A、Ig M水平升高,且高于对照组(P<0.01)。治疗后2组SF-36各项评分均较治疗前显著升高(P<0.05或P<0.01),且观察组显著高于对照组(P<0.01)。结论:穴位贴敷联合常规抗结核治疗可改善复治性肺结核患者肺功能,并提高其体液及细胞免疫功能,同时有效地改善了患者的生命质量,效果确切。Objective： To investigate the effects of acupoint application combined with conventional anti-tuberculosis therapy on quality of life and immune function in patients with recurrent pulmonary tuberculosis. Methods： A total of 54 patients with recurrent pulmonary tuberculosis who were admitted and treated in our hospital from May 2014 to May 2016,were divided into control group（ n = 26） and observation group（ n = 28） according to the treatment plan. The control group was treated with 3 HRZES/9 HRE standard chemotherapy regimen,and the observation group received additional Chinese materia medica acupoints application. 4 weeks is 1 treatment course and treating for 2 courses. The clinical curative effects of patients in the 2 groups were recorded and compared; the changes of lung function and immune function were detected and compared; the quality of life of the 2 groups of patients was assessed using the Simple Health Measurement Scale（ SF-36）. Results： The absorption rate of focus in the observation group was 75. 00%,which was significantly higher than 46. 15% in the control group（ P 0. 05）. After treatment,FEV1,FVC and FEV1/FVC in the 2 groups were significantly higher than those before treatment（ P 0. 05 or P 0. 01）,and the observation group was significantly higher than that of the control group（ P 0. 01）. After treatment,the levels of CD3~＋,CD4~＋and CD4~＋/CD8~＋in the observation group were significantly higher than those before treatment and the control group（ P 0. 05 or P 0. 01）; the level of CD8~＋was lower than that before treatment and the control group（ P 0. 01）; the levels of immunoglobulin Ig G,Ig A and Ig M were higher than before treatment and the control group（ P 0. 01）. After treatment,the scores of SF-36 in the 2 groups were higher than those before treatment（ P 0. 05 or P 0. 01）,and the scores of SF-36 in the observation group were significantly higher than those in the control group（ P 0. 01）.
