中药熏蒸治疗血液透析患者尿毒症性皮肤瘙痒临床护理观察及机制探讨

摘　　要：目的 :观察中药熏蒸治疗血液透析患者尿毒症性皮肤瘙痒的临床护理疗效,并探讨其可能机制。方法 :将90例患者随机分为对照组和治疗组各45例,均采用常规治疗,治疗组在此基础上予中药熏蒸,两组在治疗2周后观察疗效。结果 :治疗组较对照组明显缓解皮肤瘙痒程度;下调肌酐、血磷、血钙、PTH、组胺水平;并下调CRP、IL-1β、IL-6蛋白表达水平(P<0.05)。结论 :在常规治疗的基础上,中药熏蒸通过改善血液透析患者体内微炎症水平,下调肌酐、血磷、血钙、PTH、组胺水平从而缓解皮肤瘙痒。Objective ：To observe the clinical nursing effect of traditional Chinese medicine fumigation on uremic pruritus in hemodialysis patients and to explore its possible mechanism.Methods ：90 patients were randomly divided into control group and treatment group 45 cases,all were treated with routine treatment,the treatment group was treated with traditional Chinese medicine fumigation,two groups were observed after 2 weeks treatment.Results ：The degree of skin Itching was significantly relieved in the treatment group than in the control group.The levels of creatinine,serum phosphorus,serum calcium,PTH and histamine were reduced.The expression of CRP,IL-1 β and IL-6 protein were reduced（P〈0.05）.Conclusion ：On the basis of routine treatment,traditional Chinese medicine fumigation can improve the level of micro inflammation in hemodialysis patients,and reduce serum creatinine,serum phosphate,serum calcium,PTH and histamine levels to relieve skin itching.
