通滞苏润江片治疗类风湿关节炎患者疗效及对炎性因子及内皮细胞功能的影响

摘　　要：目的探讨通滞苏润江片对类风湿关节炎患者的临床疗效，并探究通滞苏润江片对炎性因子及内皮细胞功能的影响。方法选取在延安大学附属医院就诊的类风湿关节炎患者100例，随机分为实验组及对照组（50例/组）。对照组给予西药治疗，同时服用安慰剂，实验组将对照组的安慰剂替换为通滞苏润江片治疗。治疗6个月后，检测血沉（erythrocyte sedimentation rate，ESR）、C反应蛋白（C-reactive protein，CRP）、类风湿因子（rheumatoid factor，RF）、肿瘤坏死因子（tumor necrosis factor，TNF-α）、白细胞介素（interleukin，IL）-1、血管内皮黏附分子-1（serum vascular endothelial adhesion molecule-1，sVCAM-1）、可溶性细胞黏附分子-1（soluble cell adhesion molecule-1，sICAM-1）及血管性血友病因子（von Willebrand factor，vWF）的含量，并比较对照组和实验组疗效。结果与对照组比，治疗后实验组的晨僵时间、疼痛评分、疲劳评分、关节肿胀数、关节压痛数、DAS28、CRP、RF、ESR、TNF-α、IL-1、sVCAM-1、sICAM-1及vWF均降低（均P〈0.05）。实验组疼痛评分改善明显（P〈0.05）。结论通滞苏润江片可改善类风湿关节炎患者病情，其机制可能与调控体内急性期蛋白分泌，降低炎性因子含量以及改善内皮细胞功能相关。Objective To investigate the clinical efficacy of Tongzhi Su Runjiang tablet on rheumatoid arthritis（RA） patients and its effect on inflammatory factors and endothelial cell function.Methods ALL 100 RA patients were randomly divided into experimental group and control group（50 cases in each group）. While the patients of both groups were treated with the same anti-RA medicine for 6 months, patients of the experimental group were given Tongzhi Su Runjiang at the same time. Erythrocyte sedimentation rate（ESR）, C-reactive protein（CRP）, rheumatoid factor（RF）, tumor necrosis factor（TNF-α）, interleukin（IL） -1, serum vascular endothelial adhesion molecule-1（sVCAM-1）, soluble cell adhesion molecule-1（sICAM-1） and von Willebrand factor（vWF）, and the therapeutic effect were measured after the treatment.Results Compared with the control group, the morning stiffness time, pain score, fatigue score, joint swelling, joint tenderness, DAS28, CRP, RF, ESR, TNF-α, IL-1, sVCAM-1, sICAM-1 had significantly decreased than those in the experimental group（all P〈0.05）. In the experimental group, the cases of ACR50 and ACR70 were significantly increased（P 〈0.05）.Conclusion Tongzhi Su Runjiang tablet can significantly improve the condition of RA patients, and its mechanism may be involved in regulating the secretion of inflammatory factors and improving endothelial cell function.
