外周血CD4^+CXCR5^+Tfh样细胞在ITP患者的变化及其临床意义

摘　　要：目的观察新型辅助B细胞的CD4^+T辅助细胞亚群-滤泡辅助性T细胞(TFH)在免疫性血小板减少症(ITP)中的表达,探究其在ITP发病中的作用及机制。方法采集2013年10月至2014年10月期间在本院血液科住院的24例ITP患者作为研究组,同时采集门诊同期体检的26名健康体检者作为对照组,采用流式细胞术(FCM)的方法检测所取得的两组标本外周血中单个核细胞(PBMC)的CXCR5^+CD4^+Tfh细胞/CD4^+T细胞的百分率。分析CXCR5^+CD4^+Tfh细胞/CD4^+T细胞的百分率在两组之间差异是否有统计学意义。结果研究组患者外周血单个核细胞中CXCR5^+CD4^+Tfh/CD4^+T(12.396±1.155)%,显著高于对照组受试者(9.387±0.692)%,差异有统计学意义(t=11.273,P<0.01)。结论 Tfh细胞在ITP患者外周血中比例增高,可能在ITP的发病机制中发挥重要作用,为Tfh能否成为ITP的免疫调节和干预提出新的方向提供了证据。Objective To identify the possible roles of a new CD4＋T helper cell subset that provides help of B cells, T follicular helper （Tfh） cells, in the pathogenesis of immune thrombocytopenia （ITP） .Methods Those 24 patients peripheral blood with ITP in our hospital from October 2013 to October 2014 were collected as experimental group, named A group. And peripheral blood samples of those 26 health checkup people in ourpoliclinic from October 2013 to October 2014 were collected as healthy control group, named B group. The ratio of CXCR5＋ CD4＋ Tfh/CD4＋ T cells in peripheral blood mononuclear cells （PBMC）, from A and B group were determined by flow cytometry. Analysis if there is a statistically significant difference about the ration of CXCR5~ CD4＋Tfh/CD4＋T between A group and B group.Results The ratio of CXCR5CD4＋Tfh/CD4＋T of A group was （ 12.396＋ 1.155%） significantly higher than that （9.387＋0.692%） in B group （t= 11.273,P〈0.01 ）.Conclusions The ratio of Tfh cells in PBMCs in ITP patients increases, which indicates that Till cells may play an important role in the pathogenesis and treatment of ITP.
