成人慢性鼻窦炎伴息肉型患者血清25-羟维生素D水平变化及与病变程度的相关性观察

摘　　要：目的观察成人慢性鼻窦炎伴息肉型患者血清25-羟维生素D水平变化,并对其变化规律与鼻息肉病变程度进行相关性进行评估,为治疗提供实验基础。方法收集符合诊断及排除标准的慢性鼻窦炎伴息肉型(chronic rhinosinusitis with nasal polyps,CRSwNP)患者,以同期入院的慢性鼻窦炎不伴息肉型(chronic rhinosinusitis without nasal polyps,CRSsNP)患者为对照,术前检查血清25-羟维生素D水平,进行对比。同时应用Lund-Mackay及LundKennedy评分对CRSwNP患者鼻窦病变范围及鼻息肉病变程度进行评估,并与25-羟维生素D含量变化进行相关分析。结果共计40名患者纳入研究,其中CRSwNP组18人,CRSsNP组22人。CRSwNP组血清25-羟维生素D水平显著低于CRSsNP组(P<0.05)。CRSwNP组血清25-羟维生素D水平与代表鼻息肉严重程度的Lund-Kennedy评分呈显著负相关(P<0.01),与代表鼻窦病变范围的Lund-Mackay评分不相关。结论 CRSwNP患者血清25-羟维生素D含量显著降低,含量变化与鼻息肉病变程度显著相关,25-羟维生素D对鼻息肉的发生发展可能具有调节作用。Objective To observe the serum 25-hydroxyvitamin D levels change in adult chronic rhinosinusitis with nasal polyps patients.The disease severity was assessed according to Lund-Kennedy and Lund-Mackay score.The co-herence between 25-hydroxyvitamin D levels and the scores were observed as experiment foundation for future treat-ment.Methods According to the diagnosis and rejected criterion,the chronic rhinosinusitis with nasal polyps patients were recruited,and the contemporaneous chronic rhinosinusitis without nasal polyps patients were recruited as control. The serum 25-hydroxyvitam D levels were tested and compared.In the chronic rhinosinusitis with nasal polyps patients group,the severity of disease was assessed with Lund-Mackay and Lund-Kennedy scores.The coherence between 25-hydroxyvitam D and those scores were analyzed with Pearson＇s correlation analysis.Results Total 40 patients were re-cruited in this study.There were 18 patients in chronic rhinosinusitis with nasal polyps group and 22 patients in control group.The serum 25-hydroxyvitam D levels were significantly lower than that in control group（P 〈0.05）.The coher-ence between serum 25-hydroxyvitam D levels and Lund-Kennedy scores which related to severity of nasal polyps was correlated remarkably,but no such relationship with Lund-Mackay scores which related to severity of sinusitis.Conclu-sion The 25-hydroxyvitam D levels are significantly lower in chronic rhinosinusitus with nasal polyps patients and cor-related with the severity of nasal polyps.The 25-hydroxyvitam D might have modulating effect to nasal polyps.
