海蜇蜇伤患者血清HIS5-HT及细胞因子变化的研究

摘　　要：目的研究海蜇蜇伤患者血清组胺（HIS）、5-羟色胺（5-HT）及肿瘤坏死因子-a（TNF—d）、白细胞介素-6（IL-6）的变化及临床意义。方法将北戴河医院2015—07～2017—08收治的海蜇蜇伤患者80例作为蜇伤组，再将本组分为轻、中、重型三个亚组，选择我院同期体检健康职工28例作为正常对照组。采用ELISA法测定各组血清HIS、5-HT、TNF—d、IL-6浓度，同时比较各组血清指标差异。结果各组血清HIS、5-HT、TNF-a、IL-6浓度比较，蜇伤组明显高于对照组，中、重型组明显高于轻型组，重型组明显高于中型组（P均〈0．01），但轻型组与对照组相比较仅轻度升高，差异无统计学意义（P〉0．05）。结论海蜇蜇伤患者血清HIS、5-HT及细胞因子越高越，病情越重。早期应用抗炎、抗HIS药物对改善海蜇蜇伤患者的预后可能有重要的意义。Objective To study the changes and clinical significance of the levels of serum histamine （ HIS）, 5 - hydroxy tryptamine （ 5 - HT）, tumor necrosis factor - a （ TNF - a ） and interleukin - 6 （ IL - 6） in patients with jellyfish sting. Methods Eighty patients receiving treatment in the Beidaihe Hospital from July 2015 to August 2016 were divided into mild, moderate and severe sting group, and 28 healthy workers in the hospital were taken as normal control group. The levels of serum HIS, 5 - HT, TNF - ot and IL - 6 were measured by ELISA, and the difference were analyzed. Results The levels of serum HIS, 5 - HT, TNF - c~ and IL - 6 in the experimental group were significantly higher than those in the control group, with statistical significance （ P 〈 0.01 ）. The levels of serum HIS, 5 - I-IT, TNF - oL and IL - 6 in the moderate and severe group were significantly higher than those in the mild group, with statistical significance （P 〈 0.01 ）. The levels of serum HIS, 5 - HT, TNF - c~ and IL -6 in the severe group were significantly higher than those in the moderate group, with statistical significance （P 〈 0.01 ）, but there were no significant differences between the mild group and the control group （P 〉 0.05）. Conclusion These results suggest that the higher the levels of serum HIS, 5 - HT and cytokine, the severer degree of jellyfish sting. Early application of anti - inflammatory, anti - histamines may have important implication for improving the prognosis of patients with jellyfish sting.
